Akut Hafif Biliyer Pankreatitte Kolesistektomi Ne Zaman Yapılmalı?

GİRİŞ ve AMAÇ: Akut biliyer pankreatit(ABP) atağı sonrası tekrarlayan biliopankreatik olayları önlemek amacıyla definitif tedavi kolesistektomidir. Özellikle ABP vakalarının %80 ini oluşturan hafif biliyer pankreatitte kolesistektominin zamanlaması konusunda farklı yaklaşımlar mevcuttur. Bu çalışmamızda, hafif biliyer pankreatit atağı sonrası erken(ilk yatışta) ve interval (4 - 12 hafta sonra) dönemde laparoskopik kolesistektomi yapılan hastaların sonuçlarını karşılaştırmayı amaçladık GEREÇ ve YÖNTEM: Çalışmamızda, kliniğimizde Ocak 2010 - Aralık 2014 tarihleri arasında kliniğimizde ABP tanısı ile laparoskopik kolesistektomi uygulanan hastalar retrospektif olarak değerlendirildi. Grup 1(n: 23), erken( ilk yatışta) kolesistektomi yapılan hastalar, grup 2(n: 54) geç( 4 - 12 hafta sonra) kolesistektomi uygulanan hastalardan oluşturuldu. Gruplar; demografik veriler, operasyon süresi, komplikasyon, mortalite ve tekrarlayan biliopankreatik olaylar açısından karşılaştırıldı. BULGULAR: Çalışmamıza hafif akut biliyer pankreatit tanısı ile kolesistektomi uygulanan 77 hasta dahil edildi. Tedavi kılavuzlarına uygun olarak erken kolesistektomi hastaların sadece %29,8(23/77) uygulandı. Erken dönem (Grup 1) ve interval dönemde(Grup 2) kolesistektomi uygulanan gruplar arasında operasyon süresi, konversiyon ve komplikasyonlar açısından istatistiksel anlamlı farklılık saptanmadı(sırasıyla p: 0,210, p: 0,540, p: 0,307). Grup 2 de hastaların %14,8 inde ( 8/54) tekrarlayan biliopankreatik olaylar görüldü. Grup 1 de hastanede kalış süresi istatiksel anlamlı olmak üzere daha kısa idi(p: 0,001). TARTIŞMA ve SONUÇ: Hafif ABP atağı sonrası erken dönemde laparoskopik kolesistektomi yapılması güvenli ve tekrarlayan biliopankreatik olayları önlemede etkin bir yaklaşımdır. Ancak ABP tedavisinde erken kolesistektomi klinik pratikte sınırlı sayıda hastaya uygulanmaktadır. Bu yaklaşımın nedenleri ve bu sorunun giderilmesine yönelik öneriler sunan çalışmalara ihtiyaç vardır.

When Cholecystectomy Should Be Done in Acute Mild Biliary Pancreatitis?

INTRODUCTION: After an attack of acute biliary pancreatitis (ABP) in order to avoid repetitive biliopancreatic events, the definitive treatment is cholecystectomy. In particular, different approaches to the timing of cholecystectomy in ABP which was constituted 80% of mild forms of pancreatitis. In this study, we aimed to compare the outcomes of patients after mild biliary pancreatitis attacks who underwent early (in the first admission) and late (after 4 - 12 weeks) laparoscopic cholecystectomy. METHODS: In this study, patients who underwent laparoscopic cholecystectomy in our clinic between January 2010 and December 2014 with the diagnosis of ABP were retrospectively evaluated. Patients divided into two group; in group 1 (n = 23) early (in the first admission) cholecystectomy and in group 2 (n = 54) late (after 4 - 12 weeks) cholecystectomy was performed. Groups were respectively compared in terms of demographic data, operation time, complications, mortality and recurrence of biliopancreatic events. DISCUSSION and CONCLUSION: In mild ABP attack early period laparoscopic cholecystectomy is a safe and effective treatment choice for preventing recurrent biliopancreatic events. However, early cholecystectomy in the treatment of ABP is applied to a limited number of patients in clinical practice. New studies are need for the reasons of this approach and suggestions to fix this problem.
