Astım Patogenezine Yeni Bir Bakış Açısı: Epitel Bariyer Fonksiyonu ve Disfonksiyonu

ÖzHavayolu epiteli başlıca dış ve iç oratm arasında fiziksel bir bariyer olarak işlev görmektedir. Epitelyal bariyer fonksiyonu sıkı bağlar, adherans junctionlar ve desmozomların uygun hücre hücre bağlantılarıyla sürdürülür.  Astımda epitelin bariyer fonksiyonunun bozulduğu epitelyal frajilitenin arttığı görülmektedir. Epitelyal bariyer fonksiyonunda bozulma sonucu inhalen allerjenler, virüsler ve diğer çevreseletkenlerin submukozada antijen sunan hücreye erişiminin artması, Th2 tip aracılı immün yanıtı uyarılmasına ve epitelyel mezenşimal transisyon (EMT) uyarılmasındaönemli rol oynar. Genetik yapımız, astımlı epitel hücrelerinde değiştirilmiş bir sitokin salınmasının bir nedeni olabilir. Böylelikle astım patogenizinde hava yolu epi-teli bariyer bariyeri ve mediatör özellikleri ile patogenez ve tedavi yeni bir bakış açısı sağlamaktadır.
