KALP CERRAHİSİNİN İŞİTME FONKSİYONLARI ÜZERİNE ETKİSİNİN ARAŞTIRILMASI
Giriş: Ekstrakorporeal dolaşım ile gerçekleştirilen açık kalp cerrahisi ni takiben gelişen i şitme kaybı nadir olarak bildirilen bir komplikasyondur. Patogenezi tam olarak bilinmemektedir. Çal ışmamızın amacı açık kalp cerrahisi geçiren hastalarda oluşabilecek olası işitme kayıplarını tespit etmek amacıyla preoperatif ve postoperatif pür ton odiometri ölçümü sonuçlarını karşılaştırmaktır. Gereç ve Yöntem: Temmuz 2016- Mayıs 2017 tarihleri arasında açık kalp ameliyatı geçiren 21 hastada ameliyat öncesi ve sonrası pür ton odyometri testi 0.25, 0.5, 1, 2, 4, 6, 8 KHz'de gerçekleştirildi. Ölçümler operasyondan önce hazırlık döneminde ve postoperatif olarak genel durum stabilizasyonu sağlandıktan sonra iki kez yapıldı. Bulgular: Çalışmaya 21 hasta dahil edildi. Bir hasta ameliyat sonras ı erken dönemde kaybedildiği için analize alınmadı. Analizde yer alan 20 hastanın ya ş ortalaması 60.7±7.7 (min: 46, max: 76) olarak bulundu. Hastaların 4’ü (%20) kadın ve 16’sı (% 80) erkekti. Hastaların ameliyat öncesi 0.5, 1, 2, 4 KHz frekanslarındaki saf ses orta- lamaları (PTA) sağ kulakta 23.0±11.0 dB ve sol kulakta 26.3±13.0 dB olarak saptandı. Tüm hastalarda ameliyat sonrası PTA sağ kulakta 24.5±7.7 dB ve sol kulakta 28.8±15.0 dB olarak saptandı. (Sırası ile p=0.728, %95CI: - 6.8-4.8 dB ve p=0.614, %95 CI: -9.7-5.8). Ameliyat öncesi 5 sağ kulakta (%25) hastada PTA 25 dB ve üzerinde saptanırken ameliyat sonrasında bu hastaların oranı % 40’a (n=8) yükselmiştir (p=0.375) Sol kulakta PTA 25 dB ve üzerinde olan hasta sayısı ameliyat öncesinde 8 (%40) ve ameliyat sonrasında 12 (%60) olarak bulunmuştur (p=0.219). Sol kulakta ameliyat öncesi 8 KHz de işitme eşiği 41.7±23.1 dB ve ameliyat sonrasında 53.5±23.3 dB olarak saptanmış olup i şitme e şiği anlamlı olarak artmıştır (p=0.001, %95 CI: 20.2-3.2 dB). Di ğer frekanslarda ameliyat öncesi ve sonrası işitme eşiklerinde anlamlı fark saptanmamıştır. Sonuç: Çalışmamızda açık kalp ameliyatı sonrasında i şitme kaybı gözlenmemiştir. Bu durum gelişen ameliyat tekniklerinin hemodinamik stabilizasyonu sağladığını ve nörokognitif fonksiyonları bozmadığını düşündürmektedir.
INVESTIGATION OF THE EFFECT OF CARDIAC SURGERY ON HEARING FUNCTIONS
Introduction: Hearing loss conditions after open-heart surgery via extracorporeal circulation is a rarely reported complication. The aim of the study is to compare the results of preoperative and postoperative pure tone audi- ometry measurements to determine possible hearing loss that may occur in patients who had open-heart surgery. Material and Method: Audiometric measurements were performed in 21 patients who had open-heart surgery between July 2016 and May 2017. Pure tone audiometry test was performed at 0.25, 0.5, 1, 2, 4, 6, 8 KHz. Measurements were applied before the operation and afte r the general condition stabilization stage at post- operation. Results: Twenty-one patients were included in the study. O ne of the patients was excluded from the study be- cause of the post-operation death at the early stage. The mean age of 20 patients was found as 60.7 ± 7.7 (min: 46, max: 76). 4 (20%) of the patients were female and 16 (80%) of them were male. Mean values of preoperative PTA of the patients at 0.5, 1, 2, 4 KHz frequencies were calculated as 23.0 ± 11.0 dB for t he right ear and 26.3 ± 13.0 dB for the left ear. Postoperative PTA values for all patients were 24.5 ± 7.7 dB for the right ear and 28.8 ± 15.0 dB for the left ear. (P = 0.728, p = 0.614, respectively). For the left ear, the hearing threshold was found as 41.7 ± 23.1 dB at 8 KHz pre-operation stage and 53.5 ± 23.3 dB post-operation stage, which means hearing threshold increased significantly after the operation (p = 0.001, 95% CI: 20.2-3.2 dB). There is any other detected significant difference in terms of hearing thresholds frequencies. Conclusion: In this study, no loss of hearing symptoms were observed. As a result, developing surgical tech- niques provide hemodynamic stabilization and do not impair neurocognitive functions
