Tip 2 diyabetli hastalarda HbA1c ile lipidler ve lipoprotein (a) düzeyleri arasındaki ilişkinin araştırılması
Çalışmamızda, tip 2 diyabetli hasta gruplarıyla sağlıklı kontrol grubu arasında lipoprotein(a) (Lp(a))ve lipid düzeyleri karşılaştırıldı. Kan şekeri ve HbA1c değerleri çok yüksek seyreden hasta grubu 6 ay süreyle takip ve tedavi edildi. HbA1c ve kan şekeri düzeyleri kabul edilebilir değerlere indirildiğinde, serum Lp(a) ve diğer aterojen lipid düzeylerinin hangi ölçüde değişim gösterdiği saptandı. Tip 2 diyabetli, yaş ortalamaları 47,94±7,76 yıl olan 80 hasta (29 erkek ve 51 kadın) çalışmaya alındı. Hastalar HbA1c değerleri %8 olmak üzere üç gruba ayrıldı, HbA1c değerleri >%8 olan 52 hastanın 6 aylık takip ve tedavi sonucunda kan şekerleri kabul edilebilir düzeylere indirildi. Çalışma kontrollü ve prospektif olarak düzenlendi. Başlangıçta, tüm gruplar arasında Lp(a) düzeyleri yönünden anlamlı bir fark olmadığı saptandı. Altı ay sonunda, Lp(a) ve trigliserid düzeyleri arasında anlamlı bir fark saptanmadı. Total kolesterol, HDL-kolesterol ve LDL-kolesterol düzeylerinde ise anlamlı azalma saptandı. HbA1c ile tüm parametreler arasında anlamlı bir korelasyon saptanmadı. Sonuç olarak, Lp(a) düzeylerinin tip 2 diyabetli hastalarda sağlıklı bireylerden farklı olmadığı, diyabetteki kan şekeri ve lipid metabolizmasında yapılan iyileştirmelerin Lp(a) düzeylerini etkilemediği, hastalarımızda genel olarak diyabette görülen dislipidemi özelliklerinin var olduğu, diyabetin medikal tedavisinin yanısıra diyetin ve diyetteki yöresel özelliklerin de hastaların lipid profili üzerinde etkili olduğu saptanmıştır.
Investigation of the correlation between HbA1c and lipid and lipoprotein (a) levels in patients with type 2 diabetes mellitus
In this study, we compared the lipid and Lipoprotein(a) levels in patients with type 2 diabetes mellitus with the patients in the control group. A group of patients with high serum glucose and HbA1c levels was followed up for six months following medical treatment. With the normalisation of blood glucose and HbAk levels in this period, the difference in serum Lp(a) and other atherogenic lipid levels were determined. Eighty patients (29 males and 51 females) with type 2 diabetes mellitus, aged 47.94 ± 7.76 years, were included in the study. Patients were divided into three groups depending on HbA1c levels (< 6%, 6-8% and >8%). Blood glucose levels were normalised at six months following treatment in 52 patients with HbA1c levels >8%, The study was designed as a controlled and prospective study. Prior to treatment, there were no significant differences in Lp(a) levels in all of the groups. After six months, there were still no significant differences in Lp(a) and triglyceride levels. However, there was a significant decrease in total cholesterol, HDL-cholesterol and LDL-cholesterol levels. Besides, there was no correlation between HbA1c levels and all other parameters. We conclude that there was no significant difference in Lp(a) levels in patients with type 2 diabetes and the patients in the control group. In addition, the improvement in serum glucose levels and lipid metabolism in diabetes did not affect Lp(a) levels as the lipid profiles of our patients were similar to the characteristics of dyslipidemia in diabetes. Finally, the lipid profile of patients with type 2 diabetes mellitus was also found to be affected by the regional differences in diet besides medical treatment.
