Sıklıkla kullanılan klinik ve laboratuvar bulgularının akut apandisit ve over kist rüptürünün ayırıcı tanısındaki yeri
Kadınlarda preoperatif akut apandisit tanısı erkeklere göre daha düşük bir doğruluk koyulabilmektedir. Bu çalışma, genç kadınlarda akut apandisit ile en çok karışan jinekolojik patoloji olan över kist rüptürleri ile akut apandisitin ayırıcı tanısına yardımcı olmak amacıyla planlandı. Acil serviste akut apandisit ön tanısı alınan ve operasyonda akut apandisit (Grup 1), perfore apandisit (Grup 2) ve över kist rüptürü (Grup 3) tesbit edilen 57 kadın hastanın pre-operatif iştahsızlık, kusma, hassasiyet, defans, rebound, ayakta direkt karın grafisinde lokalize ileus bulgusu, lökositoz (>9000/mm3) ve koltukaltı ısısı (37.8 C°) karşılaştırıldı. Üç grup arasında iştahsızlık, kusma, hassasiyet, rebound, ayakta direkt karın grafisinde lokalize ileus bulguları açısından fark tesbit edilemedi (p>0.05). The role of frequently used clinical and laboratory findings in the differential diagnosis of acute appendicitis and ruptured ovarian cyst
Preoperative diagnosis of acute appendicitis in women is associated with a lower accuracy rate than that in men. In this study 57 women initially diagnosed to have acute appendicitis in the emergency room and then confirmed to have acute or perforated appendicitis and ruptured ovarian cysts were investigated. Loss of appetite, vomiting, tenderness, muscular resistance, rebound tenderness, radiologically air and fluid levels localized to the caecum, leukocytosis and raised axillary temperature levels were compared among three groups. There was no difference in loss of appetite, vomiting, tenderness, rebound tenderness, radiologically air and fluid levels localized to the caecum among the groups (p>0.05). Muscular resistance in acute appendicitis group, leukocytosis in acute and perforated appendicitis groups and raised axillary temperature in perforated appendicitis group were significant findings (p
