İnterstisyel sistitin patogenezi üzerine histokimyasal ve immunhistokimyasal bir çalışma
înterstisyel sistit mesanenin etiyolojisi bilinmeyen inflamatuar bir hastalığıdır. Klinik olarak kompleks boşaltım semptomlarını ve bazı sistoskoplk bulguları içermektedir, înterstisyel sistit tanısı klinikopatolojik bir tanıdır. Histopatolojik olarak interstisyeı sistitin " ülserli" ve "ülserli olmayan" olmak üzere iki formu bulunmaktadır. Bu çalışmanın amacı, înterstisyel sistitin patogenezini, klinik ve patolojik tanı kriterlerini ortaya koymak, hitopatolojik olarak ayrılan 2 formun temel farklılıklarını göstermektir. Bu amaçla klinik ve patolojik olarak interstisyel sistit tanısı almış 10 olgunun sistoskopik biopsi veya TUR uygulanan doku örnekleri histokimyasal ve immünhistokimyasal yöntemlerle ve klinik özellikleri ile birlikte değerlendirilmiştir. Ülserli olgularda inflamatuar hücrelerden plazma hücrelerinin ülsersiz formlara göre daha yoğun olduğu gözlenmiştir ( p0.05). Bu bulgularla interstisyel sistit tanısının klinik ve patolojik incelemelerin birlikte değerlendirilmesi ile mümkün olabileceği düşünülebilir. Olgularımızda T lenfositlerinin ve plazma hücrelerinin baskın oluşu, mast hücrelerinin bazı olgularda daha yoğun izlenmesi, diğer çalışmaların bulgularıyla birlikte değerlendirildiğinde interstisyel sistitin patogenezinde immünolojik mekanizmaların rol oynadığını düşündürmektedir.
An histochemical and immunohistochemical study on the pathogenesis of interstitial cystitis
Interstitial cystitis is a chronic inflamatuar bladder disorder of unknown etiology. This disorder includes complex clinical voiding symptoms and cystoscopic findings. Interstitial cystitis is a clinicopathologic diagnosis. Histopathologically there are two forms of interstitial cystitis as " ulcerated " and"non-ulcerated" type. Objective o1 this study is to investigate the pathogenesis, clinical and pathological diagnostic criteria and the main differences of two subtypes of this disorder . For this purpose cystoscopic biopsies or TUR materials of 10 cases diagnosed as interstitial cystitis clinically and pathologically were evaluated by histochemical ana immunhistochemical methods with their clinical findings. Plasma cells were observea more intensively in ulcerated than non-ulcerated forms ( p0.05). This may show that evaluation of both pathological and clinical findings leads to the diagnosis of interstitial cystitis. Observation of Increased number of 7 cells and plasma cells in our cases and increased mast cells in some cases, when the result of the other studies are also considered, may show that immunologicai mechanisms plays a role in the pathogenesis of interstitial cystitis.
