Akut bakteriyel menenjit geçiren çocuklarda uzun süreli sekellerin araştırılması
Amaç: Bu çalışma akut bakteriyel menenjit geçirmiş çocuklarda uzun sürede işitme kaybı, nörolojik problem sıklığı, okul başarısı ve zeka bölümü değişikliklerini araştırmak amacıyla yapıldı. Yöntem: Çalışma, hastalık sırasında yaşları 2 ay-12 yıl olan 35 çocukta, akut hastalıktan 8 ay- 6 yıl (ortalama 3.7 yıl) sonra yapıldı, işitme kaybı pür ton odiyogram ve timpanogram ile değerlendirildi. Aile görüşmeleri ve olgunun değerlendirilmesi ile nörolojik problemler ve okul başarısızlıkları belirlendi. Üç olgu dışında çalışma (n=32) ve kontrol grubunun (n=30) zeka bölümleri psikolog tarafından tespit edildi. Bulgular: Bütün çocukların timpanogramları normal idi, pür ton odiyogramda % 32.2 oranında sensörinöral tipde işitme kaybı saptandı. Vakaların dördünde (% 11.4) okul başarısızlığı, ikisinde (% 5.7) kalıcı baş ağrısı, ikisinde (% 5.7) tekrarlayan konvülziyon ve birinde (% 2.8) motor kayıp olduğu belirlendi. Zeka bölümleri çalışma grubunda (88.0±12.5) kontrol grubuna göre (95.8+6.3) düşük bulundu, işitme kaybı olan çocuklarda, opistotonus ve konvülziyonların daha uzun sürdüğü belirlendi. Sonuç: Akut bakteriyel menenjit sonrası işitme kaybı, nöropsikolojik sekeller ve düşük zeka düzeyleri oluşabilmektedir. Sekellerin ve etkilerinin azaltılması amacıyla aşı programları ve hastalığın tedavisi sırasında rutin deksametazon uygulanması yanında, henüz hastalar hastaneden ayrılmadan sekellerin tespit edilerek rehabilite edilmesi önemli yararlar sağlayacaktır.
The investigation of long term sequelae of childhood acute bacterial meningitis
Objective: In this study, it was aimed to determine the long-term hearing loss, neurological problems and changes in school performance and intelligence quotient (IQ) scores of the children who had recovered acute bacterial meningitis. Methods: Thirty-five children, aged two months to 12 years in diseased state, who had recovered from acute bacterial meningitis 8 months to 6 years earlier (mean 3.7 year) were investigated. Pure tone odiometry and tympanometry were performed in order to determine audiological abnormalities. IQ scores were measured (n=32) and they were compared to controls (n=30) by psychologist. Results: Tympanograms were normal in all children, sensorineural hearing loss was observed in 10 (32.2 %) of 31 children. Four children (11.4 %) had school disabilities, two (5.7 %) had permanent headache, two (5.7 %) had recurrent convulsion and one (2.8 %) had motor deficit. Intelligence quotient scores of the study groups (88.0±12.5) was lower than those of the control (95.8±6.3). It was determined that the children who had sensorineural hearing loss were suffered longer term convulsion and more opustotonus. Conclusion: Acute bacterial meningitis may lead to hearing loss, neuropsychological sequelaes and low intelligence quotient. In order to decrease these sequelaes and their effects, vaccination programs and adjunctive therapy with dexamethasone should be made. In addition, it may be useful to determine the sequelaes and rehabilitate them before hospital discharge.
