Tiroidektomi sonrası geç komplikasyonlar ve nüks

Amaç: Bu çalışma tiroidektomilerden sonra ortaya çıkan geç komplikasyonlarm sıklığını belirlemek için yapıldı. Yöntem: S.Ü.Tıp Fakültesi Genel Cerrahi kliniğinde 1990-1996 yılları arasında tiroidektomi uygulanan 339 hasta çalışmaya alındı. Hastaların TSH, Ta, T/tve serum kalsiyum değerleri tayin edildi. Bütün hastalar indirekt laringoskopi ve ultrasonografi ile değerlendirildi. Bulgular: Hastaların % 15'inde hipotiroidi, % 5.6'smda nüks, % 4.1'inde hipokalsemi ve % 2.4'ünde vokal kord paralizisi belirlendi. Toplam komplikasyon oranı % 27.1 olarak bulundu. Sonuç: Komplikasyonlardan kaçınmak için operasyon endikasyonunun iyi belirlenmesi, hasta için en uygun operasyonun yapılması, nervus laryngeus inferiorun gözlenmesi ve hormon replasmanı yapılması gerektiği kanaatine varıldı.

Late complications and recurrence after thyroidectomies

Objective: This study was performed to find out the late complications of thyroidectomies. Methods: Three hundred thirty-nine patients on whom thyroidectomy operations were performed between 1990 - 1996 in the department of general surgery, Faculty of Medicine, University of Selçuk were included in this study. Plasma TSH, Ta, T4 and serum calcium levels of the patients were measured. All patients were screened by ultrasonography and indirect laryngoscopy. Results: The incidences of late complications were as follows: Hypothyroidism in 51 (15%), recurrence in 19 patients (5.6%) hypocalcemia in 14 (4.1%) and vocal cord paralysis in 8 (2.4%). The total complication ratio was 27.1%. Conclusion: It is concluded that in order to reduce the risk of any complication surgical indications should be limited, a suitable operative procedure should be performed, recurrent laryngeal nerve should be dissected and postoperative hormone replacement should be done.
