Laparoskopi çağında safra taşı hastalığı ve umbilikal herni birlikteliğinin önemi

Amaç: Ortak risk faktörlerine sahip kolelitiyazis ve umbilikal herdi, aynı hastada cerrahın karşısına çıktığında bazı teknik sorunlara yol açabilmektedir. Buna karşın kolelitiyazisli hastalarda umbilikal herdi görülme insidansını araştıran çalışmaların sayısı oldukça azdır. Çalışmamızda, kliniğimizde laparoskopik kolesistektomi uyguladığımız hastalarda umbilikal herdi görülme oranının belirlenmesi ve buradan yola çıkılarak kolelitiyazis ve umbilikal herninin aynı hastada birlikte görülmesinin neden olabileceği sorunların tartışılması amaçlanmıştır. Yöntemler: 2006 ve 2012 yılları arasında laparoskopik kolesistektomi yapılan 886 semptomatik kolelitiyazis hastasının kayıtları incelendi. Bulgular: Sekiz yüz seksen altı hastanın 204unde (%23.02) umbilikal herdi birlikteliği tespit edildi. Bu hastalardan 163u kadın, 24u erkekti. Ortalama beden kitle indeksi umbilikal hernisi olan hastalarda 32.6 kg/m², olmayan hastalarda 28.1 kg/m² idi. Sonuç: Laparoskopik kolesistektomi yaptığımız hastalarda umbilikal herdi oranı diğer çalışmalara Gore yüksek çıkmıştır. İki hastalığın birlikteliği bazı teknik sorunlara neden olabileceğinden umbilikal herninin laparoskopik kolesistektomi öncesinde fark edilmesi önemlidir. (Gazi Med J 2012; 23: 59-61)

The significance of the coexistence of cholelithiasis and umbilical hernia in laparoscopy era

Objective: Cholelithiasis and umbilical hernia, which have common risk factors, may cause some technical problems to the surgeon when encountered in the same patient. However, there are only a few studies investigating the incidence of umbilical hernia in patients with cholelithiasis. Our purpose is to define the incidence of umbilical hernia in patients undergoing laparoscopic cholecystectomy in our clinic, and to discuss the potential problems that can be caused by the coexistence of cholelithiasis and umbilical hernia in the same patient. Methods: The records of 886 patients who underwent laparoscopic cholecystectomy for symptomatic cholelithiasis between 2006 and 2012 were investigated. Conclusion: The incidence of umbilical hernia in patients undergoing laparoscopic cholecystectomy was higher in our study than in previous studies. Since the coexistence of the two diseases may cause technical problems, it is important to diagnose umbilical hernia before the operation in patients undergoing laparoscopic cholecystectomy.
