Kalp-damar sistemi muayenesi: En eski fakat eskimeyen bir tanı yöntemi
Amaç: Kalp-damar sistemi muayenesinin önemini vurgulamayı amaçlayan bu çalışmada, doğumsal kalp hastalığı olan çocuklarda ilgili üfürümün ne zaman duyulduğu ve doğumsal kalp hastalığı tanısının ne zaman konulduğu araştırılmıştır. Gereç ve Yöntemler: Ocak 2008 ve Ocak 2009 arasında çalışma merkezine gönderilen ve kalp hastalıklarına geç tanı konulduğu düşünülen 284 çocuk geriye dönük incelenmiştir. Bulgular: Çalışmada çocuklarda en sıklıkla belirlenen doğumsal kalp hastalıkları sırasıyla atrial septal defekt (%33.1), patent duktus arteriozus (%19.0) ve aort koarktasyonu (%10.6) idi. Çalışma kapsamına alınan çocukların çoğu (%72.5) İç Anadolu Bölgesinde yaşarken onbeşi (%10.6) Güneydoğu Anadolu Bölgesinden ve onüçü (%9.2) Karadeniz Bölgesinden gelmekteydi. Ortalama üfürüm duyulma yaşı 8.0±5.2 yıl olarak hesaplanırken üfürümün duyulması ve doğumsal kalp hastalıklarına tanı konulması arasında geçen ortalama süre 6.6±4.9 yıl idi. Aort koarktasyonlu çocuklarda ortalama tanı yaşı 9.2±4.7 yıl olarak bulundu. Çalışma kapsamında incelenen erkek çocuklar, Akdeniz Bölgesinden sevk edilen çocuklar ve aorta koarktasyonu saptanan çocuklar için hesaplanan tanısal gecikme süreleri anlamlı olarak daha uzun bulundu (sırasıyla p=0.003, p=0.040 ve p=0.019). Doğumsal siyanotik kalp hastalığı olan çocuklarla karşılaştırıldığında siyanotik olmayan kalp hastalığı bulunan çocuklardaki tanı yaşının anlamlı düzeyde daha büyük olduğu görüldü (p=0.001). Sonuç: Teknolojinin sağlık sektörünün hizmetine sunulmasıyla birlikte geleneksel değerlendirme yöntemlerinden olan öykü alma ve fizik muayene eskisi kadar önemsenmemektedir. Ancak fizik muayene, doğumsal kalp hastalıklarının erken tanısında en basit ve etkili yöntem olarak geçerliliğini korumaktadır. Çocuk hekimleri, sadece kalp seslerini dinleyerek, çift taraflı arter nabızlarını değerlendirerek ve kan basıncını ölçerek, geri dönüşsüz sağlık sorunlarına ve ciddi bir ekonomik yüke neden olabilecek doğumsal kalp hastalıklarının erken dönemde belirlenmesini sağlayabileceklerini unutmamalıdır.
Cardiovascular examination: The oldest but still an up-to-date diagnostic method
Objective: The present study aims to emphasize the significance of cardiovascular system examination. Materials and Methods: The present study retrospectively surveys 284 children who were referred to the study center between January 2008 and December 2009 and in whom the diagnosis of congenital cardiac anomaly was delayed. Results: The most frequently detected congenital cardiac anomaly was atrial septal defect (ASD, 33.1%) which was followed by patent ductus arteriosus (PDA, 19.0%), coarctation of aorta (CA, 10.6%) and ventricular septal defect (VSD, 9.2%) respectively. The most of the patients were from central Anatolia (72.5%), 15 of them were from southern Anatolia (10.6%) and 13 of them were from the Black sea region. Cardiac murmurs were detected at an average age of 8.0±5.2 years and the diagnosis of congenital cardiac anomaly was made after 6.6±4.9 years in average. The average age for detection of CA was found to be 9.2±4.7 years. The diagnostic delay span was significantly longer in the male children (p=0.003), children coming from the Mediterranean region (p=0.040) and children with CA (p=0.019). When compared with cyanotic malformations, non-cyanotic cardiac anomalies were diagnosed significantly later (p=0.001). Conclusions: As health sector eagerly adopts the advanced technology, traditional methods of clinical evaluation are much ignored. However physical examination still preserves its importance as the simplest method in diagnosis of congenital cardiac malformations. Pediatricians can specify the cardiac anomalies as early as possible and help to avoid considerable financial loss by means of physical examination.
