İntravasküler branül fraktürleri
Amaç: Hastaneye yatırılan olgularda gerekli tedavilerin uygulanabilmesi için periferik venöz kateterler ve yoğun bakım hastalarında da invaziv kan basıncı monitorizasyonu veya tekrarlayan kan gazı analizleri için arteryel kateterlerin takılması gereklidir. Ancak bu işlemler sırasında ve sonrasında çeşitli komplikasyonlar gelişebilmektedir. Bu yazıda periferik vasküler kateterizasyon sonrası, takılan branüllerin damar içerisinde kırıldığı 7 olguya ait sonuçları araştırdık. Yöntem: Gazi Üniversitesi Tıp Fakültesi Kalp Damar Cerrahisi Kliniğine, Ocak 2007 - Ocak 2008 ayları arasında periferik vasküler kateterizasyon sonrası, takılan branüllerin damar içerisinde kırıldığı 7 hasta konsulte edildi ve geriye dönük değerlendirilmesi yapıldı. Bulgular: Kateterlerin 4ü 20G 32mm, 3ü ise 18G 45mm enjeksiyon portlu radyoopak branül (intraket) yapısında idi. Branüllerin tamamı enjeksiyon portunun hemen distalindeki boyun kısımlarından kırılmış idi. Vakaların tamamı 3 günden fazla kullanılan periferik kateterlerin, yeterli verimde çalışmadığı veya tıkandığı için çıkarılırken komplike olmuştu Tüm olgularda intravasküler yabancı cisim tanısı ve lokalizasyonu ultrasonografik olarak konfirme edildi. Sonuç: Kateter bakımına özen gösterilmesi, sadece major komplikasyonları ve invaziv girişim ihtiyaçlarını azaltmakla kalmayacak, hasta konforunu bozan minör komlikasyonları ve enfeksiyonları da azaltmayı sağlayacaktır.
Intravascular branul fractures
Objective: Its necessary to be used venous catheters for medical therapy and arterial catheters for invasive blood pressure monitorization and blood gase analysis in hospitalized patients. However, during or after this period, some complications can be seen. In this article, branule fractures have been discussed in 7 cases after peripheral vascular catheterisation. Methods: In this review 7 cases which consulted to Gazi University Medical Faculty Department of Cardiovasculary Surgery during January 2007-January 2008 have been evaluated and retrograd analyzed. Results: In 4 patients, the size of the catheters were 20G-32mm, where as in 3 patients, the catheter sizes were 18G-45mm. All the catheters enjection ports were radioopaque. The 7 catheters which have been reported here were broken by the distal side of ports and all the catheters were used more than three days. Catheters were broken while taking off. When the problem occured, all the forreign bodies in intravascular area were identified and documented by using the ultrasonography. Conclusion: If the catheters keeping is doing well, some of the complications can be prevented and patients satisfaction will improve.
