Elimination of pain during removal of classical anterior ribbon gauze nasal packing

In this study, we aimed to eliminate the painful removal of tampon by introducing pantocaine into ribbon gauze prior to removal procedure Methods: Forty-eight cases with nasal septal deformity who underwent Cottle septoplasty were enrolled in this prospective study. At the end of the operation an appropriate 16G tube was inserted into each nasal cavity followed by anterior packing with ointment impregnated ribbon gauze. Tampons and tubes were left in the nasal cavity for 48 hours. Fifteen minutes prior to removal, ribbon gauze in the right and left nasal cavities were injected with 2 ml of pantocaine 1% and saline solution 0.9% respectively. After the removal, patients scored for their pain at left and right sides separately with Visual Analog Scale (VAS) (1-10) and bleeding was also recorded Results: The mean VAS' values were 3.2$pm$O.9 and 5.4$pm$1.2 for right and left sides, respectively. The difference between them was statistically significant (p 0.05). No bleeding was noted and nasal breathing was partially achieved by insertion of the tubes. Conclusion: Application of pantocaine prior to the removal of packing significantly diminished pain and discomfort. We believe that rehydration of the pack three times a day diminishes the adhesions and sensation of fullness in the nasal cavity.
