Yetişkinlerde Çok Kesitli Bilgisayarlı Tomografi (ÇKBT) ile İnsidental Saptanan Diyafragmatik Hernilerin Değerlendirilmesi

ÖZET Amaç: Erişkinlerde, Çok Kesitli Bilgisayarlı Tomografi (ÇKBT) ile insidental saptanan diyafragmatik hernilerin oranlarını saptamak, dağılım özelliklerini bulmak, özelliklerini tespit etmek ve literatür ile karşılaştırmaktır. Gereç ve Yöntem: Çeşitli klinik ön tanılarla başvuran 670 hastanın batın, 330 hastanın toraks olmak üzere toplam 1000 hastanın bilgisayarlı tomografi görüntüleri retrospektif olarak değerlendirildi. Hastaların diyafragmatik hernilerinin olup olmadığı araştırıldı. Diyafragmatik hernisi olan hastalar Morgagni, Bochdaleck ve Hiatal herni gruplarına ayrıldı. Hernilerin kendi içlerinde sıklıkları belirlendi ve herniler içeriklerine göre gruplandırıldı. Bulgular: 1000 hastanın 192 (% 19.2) sinde toplam 211 herni saptandı. Saptanan 211 herninin % 51 i morgagni , % 37 si hiatal , % 12 si bochdaleck idi. 1000 hastanın % 10.7 sinde morgagni, % 7.8 inde hiatal ve % 2.6 sında bochdaleck hernisi saptandı. Herni saptanan 192 hastanın % 88 i tek, % 11 i iki ve %1 i üç tip herniyi de içermekteydi. İki herni saptanan hastaların % 50 si morgagni ve hiatal, % 23 ü morgagni ve bochdaleck ve % 27 si bochdaleck ve hiatal hernilerini içermekteydi. Sonuç: Torakal ve abdominal hastalıklar için kullanılan ÇKBT gibi ince kolimasyon sağlayan tekniklerin kullanımının artması ile diyafragmatik hernilerin insidental tespiti kolaylaşmış, insidental saptanan herni sıklığı artmıştır. Diyafragmatik herniler yüksek oranda (% 19.2) ÇKBT ile saptanmıştır. En sık morgagni herni saptanırken bunu hiatal ve bochdaleck hernisi takip etmiştir.

Evaluation of Detected Incidental Diaphragmatic Hernia with Multislice Computed Tomography (MDCT) in The Adults

Objective: The aim is to retrospectively assess the proportion, distrubition and properties of incidentally detected diaphragmatic hernies (Morgagni, Bochdaleck and Hiatal hernies) in multislice Computer Tomography (CT) studies of adults and compare with literature. Material and Method: We retrospectively assessed CT findings of 1000 patients (670 abdomen and 330 thorax CT) admitted to with various clinical diagnosis. Primarily we investigate whether diaphragmatic hernia exists or not. The patients those have diaphragmatic hernia have been divided to three groups as Morgagni, Bochdaleck ve Hiatal hernia. After determine frequency we divided hernias to subgroups according to their content. Results: Of these patients, totally 211 hernias were detected in 192 patients (%19.2). Among these hernias, 51% Morgagni, 37% Hiatal , and 12% were Bochdaleck hernia. Patients those have hernia (192 individuals) were diagnosed with single (88%), double (11%) and triple(1%) type of hernias. Patients with double type of hernia were including Morgagni and Hiatus (50%), Morgagni and Bochdalek (23%)and Bochdalek and Hiatus hernia (27%). Conclusion: Increased using of thin sectional and low collimation cross sectional imaging techniques like multislice CT for thoracic and abdominal pathologies has eased incidental diagnosis of diaphragmatic hernia which raised their frequency. Diaphragmatic hernias were detected with highest ratio by using multisclice CT(% 19.2). The most common hernia type was Morgagni consequented by Hiatal and Bochdalek hernia, respectively.
