TAMAMEN İNTRÜZE OLMUŞ MAKSİLLER LATERAL KESİCİ DİŞİN CERRAHİ YAKLAŞIM İLE YENİDEN KONUMLANDIRILMASI: 9 AYLIK TAKİP

Bu olgu raporunda, travma nedeniyle sol maksiller santral kesici dişinde ekstrüzyon ve lateral kesici dişinde intrüzyon görülen 11 yaşındaki bir erkek çocuğun tedavi yaklaşımı sunulmaktadır. Hasta kliniğimize başvurmadan önce, hastanın maksiller santral kesici dişi başka bir diş hekimi tarafından konumlandırılmış ve dişler rijid bir tel ile splintlenmiştir. Hastanın radyografik muayenesinde sol maksiller lateral kesici dişinde 7 mm’den daha fazla intrüzyon ve mine dentin fraktürü olduğu saptandı. Gömük olan bu diş flep kaldırılmasını takiben davye ile hafifçe lükse edilerek istenilen konuma getirildi. Dişin konumlandırılmasını takiben yumuşak doku sütürlenerek dört hafta süre ile esnek bir tel ile splintleme yapıldı. Aynı seansta pulpa ekstirpe edilerek kanallara kalsiyum hidroksit ile pansuman yapıldı ve 2 hafta sonra kanal tedavisi tamamlandı. Klinik ve radyografik kontrollerde ve 9 aylık takip süresi sonunda, herhangi bir rezorbsiyon, marjinal kemik kaybı veya periapikal dokularda bir patoloji gözlenmedi. Tamamen intrüze olmuş daimi dişlerin tedavisinde cerrahi ekstrüzyon alternatif bir tedavi seçeneği olarak uygulanabilir

Repositioning of a completely İntruded Maxillary Lateral İncisor With Surgical Approach: A 9 Month Follow-Up Case

This report presents a case of severe intrusive luxation of maxillary left lateral and extrusion of central incisor of an 11-year old boy. His maxillary central incisor was repositioned and splinted with a rigid splint by another dentist. Radiographic examination revealed a completely intruded permanent maxillary left lateral incisor beyond 7 mm and enamel-dentin fracture. A flap was raised to expose the crown of underlying intruded lateral incisor followed by luxating the tooth with dental forceps and after repositioning to normal occlusion and suturing the soft tissue, tooth was stabilized and splinted with flexible splint for 4-weeks. Pulp extirpation and calcium hydroxide dressing of the root canals were carried out immediately. Definitive root canal treatments were accomplished 2-weeks later. Clinical and radiographic examination during recalls and after 9 months revealed no signs of progressive root resorption, marginal bone loss or periapical disease. Surgical repositioning can be suggested as a successful treatment of choice for completely intruded permanent incisors in some cases
