ERKEN ÇOCUKLUK DÖNEMİNDE GEÇİRİLMİŞ TRAVMAYA BAĞLI EKSTRA ORAL FİSTÜL OLUŞUMU: OLGU SUNUMU

Erken çocukluk çağında yaşanan travmalara bağlı olarak daimi dişlerde yapı ve şekil bozukluklarına rastlanabilmekte, bu tür dişlerde enfeksiyonunun ilerlemesi sonucu görülen pulpa nekrozu daha sık olarak karşımıza çıkmaktadır. Pulpası enfekte ve nekroze bir diş tedavi edilmediğindeyse; enfeksiyon intraoral ve ekstraoral yayılım gösterebilmektedir. Kliniğimize mentum bölgesinde ağrı şikayetiyle gelen 14 yaşındaki erkek hastanın yapılan extraoral muayenesinde mentumda fistül yoluna bağlı skar dokusu olduğu tespit edilmiş, intraoral muayenesinde, alt sağ birinci kesici dişinin labiale dilasere ve hipoplazik olduğu görülmüştür. Anamnez derinleştirildiğinde hastanın süt dişlenme döneminde alt anterior dişlerinin travmaya maruz kaldığı tespit edilmiştir. Hastanın malforme dişinin radyolojik incelemeleri sonucunda apikal bölgede radyolusensi olduğu görülmüştür. İlgili dişe kanal tedavisi uygulanıp fistül yolu intraoral müdahale ile eksize edilmiştir. Kontrol randevularında hastanın ağrı şikayetinin geçtiği ve fistülün skar dokusunun iyileşmeye başladığı tespit edilmiştir. Olgumuz, çocukluk çağında maruz kalınan travmaların periyodik muayenelerinin yapılması gerekliliğine işaret etmekte, ayrıca daha ileri dönemlerde karşılaşılan vakalarda derinlemesine bir anamnez alınarak çocukluk çağı travmalarının sorgulanmasının önemine de değinmektedir

Extra-Oral Fistula as A Result of Childhood Trauma: Case Report

Radicular cyst is generally a consequence of pulp necrosis. Radiculer cyst commonly involves the apex of the affected tooth. A fourteen-year-old male patient was referred to our clinic after showing extraoral swelling and pain on the mentum for five months which couldn’t be diagnosed and treated by medical doctors. Patients extraoral examination showed scar tissue related to fistula track and the previous interventions by medical doctors. In intraoral examination, it was noted that the right first mandibular incisor was malformed. The crown was inclined toward the labial side. The parents stated as the tooth had erupted in this form and with further questioning the parents stated that patient had a trauma history at an early age but there weren’t any medical or dental aid given at the time of the trauma. Following the complete clinic and radiographic examination of the patient it was decided the radiolucent area was related to the periapical infection of traumatized incisor. In treatment, as first step the right and left first mandibular incisors underwent root canal treatment. Following the root canal treatment the lesion and the sinus tract have been removed as surgically under local anesthesia. The follow-up appointments revealed that the clinical symptoms have pasted. The scar tissue of fistula started to heal but not completely. We like to highlight the importance of dental examination following the trauma that happened in any age. With regular follow-ups it could be possible to prevent complex complications that may occur in the years to follow up as described in the previous case
