Tam protez kullanan hastalarda doğal baş-boyun postürünün ve dik yön boyutlarının değerlendirilmesi

Diş çekimlerini takip eden alveoler kemikrezorpsiyonuna bağlı olarak mandibulanın öne veyukarı yer değiştirmesi çeneler arası ilişkilerdedeğişikliğe neden olur. Hyoid kemik ve kranioservikal ilişkiler bu değişikliklerden etkilenir.Oklüzyonun sagittal ve vertikal boyutunda dadeğişimler gözlenir. Ayrıca tam protezlerin takılmasından sonra kafa duruşunun etkilenmesi ve çiğneme kaslarının aktivitelerinin değişmesi mümkündür. Araştırmamızda alt üst tam protez taşıyanhastaların doğal baş boyun postürleri ve dik yönboyutları incelenmiştir.Bunun için Protetik Diş Tedavisi AnabilimDalı Kliniğine tam protez yaptırmak amacı ilebaşvuran hastalar içinden, en az 5 senedir aynı tamprotezi kullanmakta olan 55 yaşın üzerindeki 20bayan hasta seçilmiştir. Sefalometrik filmler dikeyyön boyutlarını da inceleyebilmek amacı ile maksimum interküspal pozisyonda ve istirahat pozisyonunda alınmıştır. Natural Head and Neck Posture and Vertical Dimension in Complete Denture Wearers

Due to alveolar bone resorption followingtooth extractions, upward and anterior rotation ofthe mandible takes place and causes changes inintermaxillar relations. The hyoid bone and craniocervical relations are affected and variations areseen in sagittal and vertical dimension of occlusion,head posture and masticatory muscle activities. Wehave examined postural and vertical dimensionchanges in edentulous patients using upper andlower complete dentures.Twenty female patients, above 55 years of age,wearing complete dentures for at least 5 years havebeen included in our study. Cephalometric filmshave been taken in maximum intercuspidation andrest position in order to examine changes in verticaldimension. Fluid level method has been used to fixate the natural head and neck posture establishedwhile the patients were placed on the cephalostat.Recordings have been taken initially with the olddentures and then immediately, 3 and 6 months following the insertion of new dentures.As a result in our study, it has been observedthat insertion of new dentures does not change natural head and neck posture. No significantchanges were observed in craniovertical SN/GV ,craniocervical angulation SN/OPT and cervicalinclination OPT/GH, CVT/GH and OPT/CVT .But it has been observed that generally the patientswere in flexion posture due to their age.Examination of vertical dimension showed thatthere was a decrase in facial height compared todentulous subjects and insertion of new dentures
