Farklı Miktarlarda Doku Kaybına Uğrayan Endodontik Tedavi Görmüş Dişlerde Fiber Post Uygulamasının Kırılma Direncine Etkisi1

kuvvetlerine dayanım açısından etkisinin değerlendirilmesi amaçlanmıştır. YÖNTEM ve GEREÇLER: Çalışmada 40 adet küçük azı dişine MOD kavite açıldıktan sonra, kanal tedavisi işlemi ‘stepback’ ve lateral kondansasyon teknikleri kullanılarak gerçekleştirildi. Dişler kalan duvar kalınlıkları açısından 2 mm ve 1 mm olmak üzere 2 gruba ayrıldı. Her bir duvar kalınlığı grubu, fiber post uygulanıp uygulanmaması açısından 2'şer alt gruba ayrıldı (n: 10). Fiber post yerleştirilmeyen alt gruplarda dişler mikrohibrit kompozit ile restore edildi. Fiber post alt gruplarında ise, dişlere fiber post simante edilip, aynı kompozit kullanılarak restorasyonları tamamlandı. Örnekler, daha sonra universal test cihazına bağlanıp basma kuvveti uygulandı. Verilerin istatistiksel analizinde, Bonferroni düzeltmeli Mann Whitney U testi kullanıldı (p0,05). Ancak, 1 mm duvar kalınlığı varlığında, fiber post uygulanan alt grup, fiber post uygulanmayan alt gruptan sayısal olarak daha yüksek direnç değerleri gösterdi (p>0,05). TARTIŞMA ve SONUÇ: Kalan duvar kalınlığının yeterli olması durumunda, fiber post uygulaması, kanal tedavili küçük azıların kırılma direncini, sadece direkt restorasyon yapılmış dişlerdekine benzer şekilde etkilemektedir. Ancak duvar kalınlığının yetersiz olması durumunda fiber post uygulaması kanal tedavisi görmüş küçük azı dişlerinin direncini arttırabilmektedir.

Evaluation Of The Influence Of Fiber Post Insertion On The Fracture Resistance Of Endodontically Treated Teeth With Various Amounts Of Tissue Loss

INTRODUCTION: The influence of fiber post insertion on the fracture resistance of endodontically treated premolar teeth with various amounts of tissue loss was evaluated. METHODS: Forty extracted premolars were used. After preparing MOD cavities, endodontic therapy was performed using the step-back and lateral condensation techniques. The teeth were divided into 2 groups according to the remaining wall thickness which were 2 mm (A) and 1 mm (B). Afterwards, each wall thickness group was subdivided into 2 subgroups depending on whether fiber posts were inserted or not (n: 10). After restoration procedures, specimens were attached to a universal testing machine and compressive force (kN) was applied. Data were analyzed with Mann Whitney U test with Bonferroni correction(p0,05). However, in the presence of 1 mm wall thickness, the fiber-post-treated subgroup showed higher but statistically not significant fracture resistance in comparison to the subgroup without fiber post application. DISCUSSION AND CONCLUSION: When the residual cavity wall thickness is sufficient, fiber post application shows similar fracture resistance with the direct composite restoration in endodontically treated premolars. On the other hand, fiber post application may increase the fracture strength of endodontically treated premolars when the remaining wall thickness is compromised.
