Warfarin kullanımına bağlı intestinal obstrüksiyon yapan intramural ince barsak hematomu

İntramural ince barsak hematomu sıklıkla antikoagulan tedaviye bağlı meydana gelen nadir bir komplikasyon olup genellikle ameliyata gerek kalmadan destek tedavi ile iyileşme sağlanabilir. Barsakta gelişen nekroz, perforasyon ve tam obstrüksiyon gibi acil durumlarda cerrahi müdahale gerekebilir. Bu çalışmada, antikoagulan kullanımına bağlı spontan intramural ince barsak hematomu nedeniyle proksimal ince barsak obstrüksiyonu gelişen 57 yaşındaki bir olguyu sunmayı amaçladık. Akut karın ağrılı hastaların medikal geçmişi özellikle antikoagülan ilaç kullanımı açısından dikkatlice sorgulanmalı ve intramural ince barsak hematomu ayırıcı tanıda akılda tutulmalıdır.

Small intestinal obstruction caused by intramural small-bowel hematoma due to the use of warfarin

Intramural small bowel hematoma is a rare complication which occurs often due to anticoagulant therapy and can be improved usually with conservative treatment without surgery. In some urgent conditions such as necrosis, perforation or complete obstruction of the bowel may require surgical intervention. In this study, we present a 57-year old case with proximal small bowel obstruction which was caused by a spontaneous intramural hematoma due to use of anticoagulant therapy. The medical history of patients with acute abdominal pain should be documented carefully in terms of the use of anticoagulant drugs and intramural small-bowel hematoma should be considered in the differential diagnosis.
