İnsüline bağımsız diabette albumin ekskresyonu ve diabetik komplikasyonlar
Bu çalışmada, insüline bağımsız 46 diabetli hastada idrarda albumin ekskresyonu ile diabetik komplikasyonlar arasındaki ilişki araştırıldı. Normoalbüminürik hastalar 1. gruba, mikroalbüminürik hastalar 2, gruba ve makroalbüminürik hastalar ise 3. gruba alındı. Birinci gruptaki hastaların mikroalbuminüri ortalamaları 9,9 ± 1,2 /µg / dk, 2. gruptakilerin 57,1 ± 18,24 µg / dk iken 3. gruptakilerin ortalamaları 1,05 ± 0,47 g / 24 saat bulundu. İkinci ve üçüncü gruptaki bu değerler istatistiksel olarak 1. gruptan anlamlı derecede farklı idi (p < 0,01). Her üç grupta da diabetik retinopati ve nöropati, iskemik kalp hastalığı, hipertansiyon ve periferik arter hastalığı sıklığı araştırıldı. Diabetik retinopati sıklığı, sırasıyla gruplarda %28,45 ve 60 idi. Diabetik nöropati sıklığı sırasıyla % 20, 45 ve 50 bulundu. Hipertansiyon % 44, 18, 70 sıklığında iken iskemik kalp hastalığı % 12, 18 ve 30' du. Periferik arter hastalığı 1. grupta %4ve3. grupta % 20 idi. Bu sonuçlar üriner albumin ekskresyonu ile önemli diabetik komplikasyonlar arasında pozitif bir ilişki olduğunu göstermektedir.
The urinary albumin excretion and diabetic complications in non-insulin dependent diabetes mellitus
In this study, the relationship between urinary albumin excretion and diabetic complications were investigated in 46 patients with non - insulin dependent diabetes mellitus. Patients with normoalbuminuria were registered to first group, patients with micro- albuminuria to second group, patients with macroalbuminuria to third group. The averages of microalbuminuria were 9,9 ± 1,2 µg /min. in 1st, 57,1 ± 18,24 µg /min. in 2nd, whereas the average of macroalbuminuria was 1,05 ± 0,47 g /24 h in 3rd. In the second and third groups, these values were significantly different than in the first group (p < 0,01). In these groups, the prevalence of diabetic retinopathy and neuropathy, ischemic heart disease, hypertension and peripheric artery disease were investigated. The prevalence of diabetic retinopathy was 28, 45, 60 % respectively in these groups. The prevalence of diabetic neuropathy was found 20, 45, 50 % respectively in these groups. While the prevalence of hypertension was 44, 18, 70 %, the prevalence of ischemic heart disease was 12, 18, 30 % respectively. The prevalence of peripheric artery disease was 4 % in first group and 20 % in third group. The results support that there is a positive relationship between the urinary albumin excretion and major diabetic complications
