Primer İntestinal Lenfanjiektazi: Diyet tedavisine yanıt veren bir olgu

İntestinal lenfanjiektazi, hipoalbuminemi bulgusu olan hastalarda çok nadir görülen protein kaybettiren enteropati nedenidir. Genellikle çocukluk ve genç erişkin yaşlarda sporadik veya ailesel olarak ortaya çıkar. Büyüme geriliği ve kronik ishal yakınması ile başvuran dokuz aylık kız bebekte periferik ödem ve hipoalbüminemi bulgusu dikkati çekti. Hipoalbuminemi ayırıcı tanısında fekal alfa 1 antitripsin düzeyi yüksekliği nedeniyle endoskopi ve biyopsi yapıldı. Endoskopide görünen beyaz plaklar ve biyopsi materyalinin histopatolojik incelemesinde geniş laktealler saptanarak intestinal lenfanjiektazi tanısı kondu. Sekonder nedenler dışlandığı için primer intestinal lenfanjiektazi düşünüldü. Hasta proteinden zengin, düşük yağlı, orta zincirli yağ asitlerinden zengin trigliserid içeren tedavi amaçlı beslenme düzenlemesine yanıt verdi.
