Granülosit koloni stimulan faktör (G-CSF) kullanımına bağlı LDH yüksekliği ve LDH izoenzim profili
Amaç: Serum LDH düzeyi multiple myeloma (MM), non-Hodgkin lenfoma (NHL), Hodgkin hastalığı gibi hastalıklarda önemli bir prognostik faktördür. Bu hastalıklarda, tedavi döneminde yükselen LDH düzeyleri hastalık progresyonunu akla getirir. Bu hastalarda G-CSF, GM-CSF gibi büyüme faktörlerinin kullanımı sırasında serum LDH düzeylerinde geçici yükselmeler bazı yayınlarda bildirilmiştir. Çalışmamızda, G-CSF'ye bağlı gelişen LDH yüksekliklerinde spesifik bir LDH İzoenzim değişikliği olup olmadığı araştırıldı. Gereç ve yöntem: Çalışmaya önceki tedavilerde gelişen nötropenileri nedeni ile G-CSF desteğine gereksinimi olan 4 MM, 4 NHL, 1ALL, 1 küçük hücreli akciğer Ca tanık olmak üzere 10 hasta alındı. Hastalardan 3'üne kemoterapiden 7 gün önce, 7'sine ise 1 gün sonra başlamak üzere 5'er gün süre ile 5$mu$g/kg G-CSF uygulandı. 0. ve 5. günlerde serum LDH düzeyleri, LDH izoenzimleri (agaroz jel elektroforez yöntemi) ve lökosit sayıları ölçüldü. Bulgular: Toplam 4 hastada belirgin lökosit artışı ile paralellik gösteren LDH yüksekliği gözlendi. Kontrol amacı ile sağlıklı 10 kişiden 5 gün ara ile alınan serum örneklerinde LDH düzeylerinde anlamlı bir değişiklik saptanmadı. G-CSF'ye bağlı LDH yüksekliği gelişen hastaların LDH izoenzimlerinde; LDH4 ve LDHS'te daha belirgin olmakla birlikte poliklonal bir artış saptandı. Sonuç: Büyüme faktörlerinin LDH profili üzerine izole etkileri; hayvan deneyleri ya da sağlıklı, büyüme faktörü kullanan, periferik kök hücre dönerlerinde yapılacak çalışmalarla daha da netlik kazanacaktır.
LDH elevation and isoenzyme profiles in use of granulocyte colony stimulating factor (G-CSF)
Objective: High LDH levels at the diagnosis in patients with various malignancies including multiple myeloma (MM), non-Hodgkin's lymphomas (NHL) and Hodgkin's disease are accepted as poor prognostic factors. In addition, increased LDH levels during chemotherapy suggest progressive disease. Hemopoietic growth factors such as G-CSF, GM-CSF which have been used utilized as supportive measurement to chemotherapy may also cause temporary elevation in LDH levels. This study was designed to find out if there is any change in a specific LDH isoenzyme type during G-CSF therapy. Material and method: 10 patients who need G-CSF support was admitted (4 MM, 4 NHL, 1 acute lymphoblastic leukemia and 1 small cell lung cancer) to our study. G-CSF 5(J$mu$g/kg was given to 3 patients starting 7 days before chemotherapy and to 7 G-CSF kullanımına bağlı LDH yüksekliği ve LDH izoenzim profili patients starting 1 day after chemotherapy and administered for 5 days. Measurement of serum LDH and LDH isoenzyme levels and WBC counts were Performed on days 0 and 5: Results: Elevation in serum LDH levels and its correlation with increase in WBC count were noted in 4 patients. No significant change in the serum LDH levels of healthy controls measured with 5 days interval, were seen. LDFI isoenzyme studies performed in patients having rise in their serum LDFI levels, resulting from G-CSF administration showed a polyclonal increment dominantly of LDH4 and LDFI5 isoenzyme types. Conclusion: The effect of growth factors on LDFI profile should be further investigated in animal studies as well as in healthy allogeneic stem cell donors who use G-CSF.
