Yüksek septal deviasyonda, septal inceltme ve bilateral spreader greftin internal nasal valv açısına etkisinin radyolojik olarak incelenmesi

Amaç: Çalışmamızda opere ettiğimiz yüksek septal deviasyonu olan hastaların internal nazal valv açısındaki düzelmeyi radyolojik olarak incelemeyi planladık.Yöntemler: Bu çalışmada yüksek septal deviasyonu olan, septal inceltme ve bilateral, açılı spreader greft ile laminar hava akımı düzeltilen 15 olgu incelenmiştir. Hepsinde otolog septal kartilaj kullanılmıştır. Septum eğri olan tarafta aşama aşama 1-2 mm lameller olarak inceltilmiş, düz bir plan sağlanmıştır. Daha sonra bilateral spreader greftler ile stabilizasyon sağlanmıştır. Preop ve postop bilgisayarlı tomografi (BT) aksiyal kesitlerle hesaplanan internal nazal valv açıları karşılaştırıldı.Bulgular: Internal nazal valv açısı düşük ve normal olan taraflarda preop ve postop ölçümler arasında istatistiksel olarak anlamlı bir fark gözlenmiştir. Açının dar olduğu tarafta ortalama 9,54±2,17 derecelik bir düzelme sağlanmış. Açının normal olduğu tarafta ortalama 2,84±1,14 derecelik bir düzelme sağlanmıştır. Preop ve postop ölçümler arasındaki farkın, açısı düşük olan tarafta, açının normal olduğu taraftan daha yüksek olduğu tespit edilmiştir. Sonuç: Çalışmamızın sonucunda septoplasti ile birlikte bilateral spreader greftin , radyolojik olarak internal nazal valv açısının artırılmasında istatistiksel olarak oldukça anlamlı artış sağlandığı tespit edilmiştir. Spreder greftin, internal nazal valve katkısı , açının dar olduğu tarafta daha fazla idi.

Radiological examination of the effect of septal thinning and bilateral spreader graft on internal nasal valve angle in high septal deviation

Objective: In our study, we planned to radiologically examine the improvement in the internal nasal valve angle of patients with high septal deviation that we operated. Methods: In this study, 15 cases with high septal deviation, septal thinning and bilateral, angled spreader graft corrected for laminar airflow were evaluated. Autologous septal cartilage was used in all of them. The septum was gradually thinned as 1-2 mm lamellae on the curved side, and a flat plan was provided. Later, stabilization was achieved with bilateral spreader grafts. Preop and postop computed tomography (CT) internal nasal valve angles calculated with axial sections were compared. Results: A statistically significant difference was observed between preop and postop measurements in the sides with low and normal internal nasal valve angle. An average correction of 9.54 ± 2.17 degrees was achieved on the side where the angle was narrow. An average correction of 2.84 ± 1.14 degrees was achieved on the normal side. It was found that the difference between preop and postop measurements was higher on the side with lower angle than the normal side. Conclusion: As a result of our study, it was found that bilateral spreader graft with septoplasty, increased the angle of the internal nasal valve radiologically, which was statistically significant. Spreder graft contributed more to the internal nasal valve at the narrow angle.
