Akut piyelonefrit ile komplike bruselloz olgusu
Ülkemizde endemik bir infeksiyon hastalığı olan bruselloz, bakteriyemi ile seyreden bir hastalık olduğundan, farklı klinik tablolara neden olabilir. Polikliniğe yüksek ateş, üşüme-titreme, sık idrara çıkma ve sol yan ağrısıyla başvuran 44 yaşında erkek hasta akut piyelonefrit ön tanısıyla yatırıldı. Yapılan incelemelerde, hastada akut bruselloza bağlı akut piyelonefrit geliştiği saptandı. Bruselloza yönelik sekiz haftalık antibiyoterapi verilen hastanın tedavi sonrasında tam iyileştiği görüldü. Brusellozun endemik olduğu bölgelerde hastalar akut piyelonefrit bulgularıyla gelebilir. Endemik bölgelerde akut piyelonefrit bulgularıyla gelen hastalarda brusella serolojisinin de bakılması, nadir görülen bu komplikasyonun tanısında yararlı olabilir.
A case of brucellosis complicated with acute pyelonephritis
Brucellosis, is an endemic disease in our country, may lead to bacteremia and cause different clinic manifestations. A 44-year-old male patient admitted to our policlinic with high fever, shivering, chilling, pollacuria, and left costovertebral pain, and interned with diagnosis of acute pyelonephritis. Subsequently, acute pyelonephritis due to acute brucellosis was detected in the clinical and laboratory examination. Antibiotic treatment for brucellosis wasgiven to patient for eight week and after treatment full recovery was seen. Patients with brucellosis may refer with symptoms of acute pyelonephritis in endemic areas for brucellosis. By using brucellos serologies to patients who have the symptoms of acute pyelonephritis may available in the diagnosis of this rare complication in areas where brucellosis is endemic.
