Tanısal yanılgıya neden olan geniş sisterna şili: Ultrasonografi ve manyetik rezonans görüntüleme bulguları
Sisterna şili, abdominal aortanın hemen sağında retrokrural alanda lokalize dilate lenfatik bir kesedir. Ana görevi, sindirilmiş yağın transportudur. Sisterna şili, oldukça değişik görünümlere sahiptir. Sisterna şilinin kalibrasyonu kontraksiyon dalgaları ile değişebilir. Otopsilerin %20'si ve lenfanjiyografik çalışmaların yaklaşık olarak yarısında görülür. Sisterna şili, genişlemiş retrokrural lenf nodunu taklit edebilir. Karakteristik yerleşimi, tübüler şekli, suya yakın dansite değerleri ve takiplerde büyüklüğünde belirgin değişiklik olmaması ayırımında yardımcı bulgulardır. Bu yazıda geniş sisterna şili saptanan bir olguda ultrasonografi ve manyetik rezonans görüntüleme bulguları sunuldu.
Large cisterna chyli causing misdiagnosis: Finding of ultrasonography and magnetic resonance imaging
The cisterna chyli is a dilated lymphatic sac in the retrocrural space usually to the immediate right of the abdominal aorta. The main function of this ductal structure is the transport of ingested fat. The cisterna chyli has a highly variable appearance. The caliber of the cisterna chyli may be altered by contraction waves. It is seen in approximately half of lymphangiographic studies and 20% of autopsies. The cisterna chyli can mimic the appearance of an enlarged retrocrural lymph node. Characteristic location, tubular configuration, attenuation values closer to water, and lack of significant change in size within a long period of time are helpful findings in differentiation. We herein present the ultrasonography and magnetic resonance imaging features of a case of large cisterna chyli.
