Geriatrik yaş grubunda görülen diffüz idiyopatik iskelet hiperostozisi: Olgu sunumu
Etyolojisi kesin olarak bilinmeyen diffüz idiyopatik iskelet hiperostozisinde spinal osteofitler dejeneratif, travmatik veya post-enfeksiyöz değişikliklere bağlı olmaksızın paravertebral ligamanların enkondral ossifikasyonu sonucunda gelişirler. Pelvis, patella, kalkaneus ve olekranonda da tutulum olmakla birlikte en sık tutulum vertebralarda [Torakal (%97), lomber (%90), servikal (%78) ve her üç segmentin birlikte tutulumu (%70)] olmaktadır. Servikal vertebra tutulumu olan olgularda disfaji, disfoni, solunum ve beslenme problemleri görülür. Bu çalışmada, C3-5 seviyesinde osteofit oluşumuyla farengeal lümeni daraltarak nefes darlığı, ses kısıklığı ve disfaji semptomlarının görüldüğü bir olgu konu ile ilgili literatürler eşliğinde gözden geçirilerek sunulmuştur.
Diffuse idiopathic skeletal hyperostosis in a geriatric patient: Acase report
The etiology of diffuse idiopathic skeletal hyperostosis is unknown. Independent from degenerative, traumatic, or post-infectious changes, spinal osteophytes develop because of enchondral ossification of the paravertebral muscles. Although it can be present in pelvis, patella, calcaneus, and olecranon; vertebral involvement is the most common location [Thoracal (97%), lumbar (90%), cervical (78%), all three segments (70%)]. Patients can experience dysphagia, dysphonia, respiratory and nutritional problems in the presence of cervical spine involvement. In this case report, a 78 year old patient who has got osteophyte formation at the C3-5 level that cause narrowing of the pharyngeal lumen and lead to respiratory distress, hoarseness, and dysphagia symptoms is presented in the light of the literature.
