Mide yerleşimli bir gastrointestinal stromal tümörün laparoskopik rezeksiyonu

Gastrointestinal stromal tümörler (GİST) sindirim sisteminin epitel kaynaklı olmayan tümörleri içerisinde en sık rastlanılan grubu oluşturmaktadırlar. En sık bulundukları lokalizasyon mide ve sonra ince bağırsaklardır. Hastalığın klinik gidişatı hakkında en önemli belirleyiciler tümör büyüklüğü, mitoz varlığı, c-kit gen mutasyonu pozitifliğidir. Günümüzde yegane küratif tedavi şekli uygun olgularda cerrahi rezeksiyon uygulanmasıdır.Laparoskopik cerrahi bazı GİST vakalarında başarıyla uygulanmaktadır. Kliniğimize melena ile başvuran, endoskopisinde yaklaşık 3 cmx3 cm’lik polipoid kitle saptanıp peroperatuar gerçek boyutunun çok daha büyük ve lümen dışına taşmış olduğu saptanan, bilobüle GİST olgusunun laparoskopik cerrahi ile tedavisi ve takibi sunulmuştur.

Gastrointestinal tumors (GISTs) represent the largest category of nonepithelial neoplasms of the gastrointestinal tract. They arise from the neoplastic degeneration of primitive mesenchymal cells and demonstrate considerable variability in their differentiation pathways. GISTs can occur in all segments of the intestine mostly occuring in the stomach and small intestine. The clinical course of GISTs is heterogeneous and the most important prognostic factors are tumor size, mitotic count, and the presence of c-kit gene mutation. Here we report a case with a huge GIST, finally resected laparoscopically, which presented with melena and a polypoid mass measuring 3cmx3cm during upper gastrointestinal endoscopy.
