İnternal karotis arterde vaskülite yol açan oftalmik zona

İntrakranyal vaskülit ender olarak herpes zoster virüs infeksiyonunun bir komplikasyonu olarak görülmektedir. Sunulan olguda 51 yaşında erkek hasta, başının sol tarafında ağrı, sol periorbital ödem, ptozis ve kemozis tablosuyla başvurmuş, kraniyal manyetik rezonans ve anjiografi tetkikleri ile ipsilateral internal karotid arter kavernöz parçasında trombotik vaskülit tanısı almıştır. Metil-prednizolon tedavisi başlanan hastada, ancak tedavinin 4. gününde sol trigeminal sinirin oftalmik dalında tipik makülopapüler herpetik döküntü gelişmiş, bunun üzerine herpetik konjonktivit ve vaskülittanısı konulmuştur. Bu olgu temelinde, postherpetik serebral vaskülitin henüz asemptomatik dönemde ortaya çıkabileceği ve komplikasyonların önlemesi açısından herpes infek¬siyonunun erken tetkik ve tedavisinin önemi vurgulanmaktadır.

Ophthalmic zoster infection presenting with internal carotid artery vasculitis

Cerebral vasculitis is a rare complication of herpes zoster virus infection. In the presented case, 51 year-old man has admitted with left-sided headache, left periorbital edema, ptosis and chemosis, and he was diagnosed as thrombotic vasculitis of internal carotid artery by cranial magnetic resonance and angiographic imagings. After the initiation of methylprednisolone therapy, typical maculopapular herpetic rash over the ophthalmic branch of trigeminal nerve appeared, the diagnosis of herpetic vasculitis and conjuctivitis was made. In this case, it is emphasized that postherpetic cerebral vasculitis can be observed at asymptomatic phase, and, as its complications could be prevented, early diagnosis and treatment of herpes zoster virus infection is important.
