İnsan Blastosöl Sıvısında Yeni Bir İnvaziv Yöntem: Blastosentez Yöntemi İle İnsan Blastokistlerindeki Blastosöl Sıvısından Preimplantasyon Genetik Tanı Analizi
Bu derlemede; vitrifikasyonda blastosöl sıvısının (blastokistlere ait sıvı) (BS) aspire edilmesi ile blastokistin gelişimini, hayatta kalma oranını ve implantasyon başarısını etkileyip etkilemediği tartışılacaktır. Blastosöl sıvısında mevcut olan genetik ürün gerçekten embriyo genomunu yansıtıp yansıtmadığı tartışma konusudur. Preimplantasyon genetik tanı (PGT) analizi için blastosöl sıvıdan elde edilen bilgi gerçekten polar cisimcik (PB), blastomer veya trofoektoderm biyopsisinden elde edilen DNA ile uyumlu sonuçları verip vermediğide bilinmemektedir.Embriyoda kavitasyon gelişimi dördüncü günde gerçekleşir. Bu süreçte iç hücre kitlesi (IHK) ve trofoektoderm (TE) hücreleri farklılaşmaya başlar. TE hücreleri blastosöl boşluğuna sodyum iyonu ile birlikte blastosöl sıvı depolamaya başlar. Bu sıvı ile blastokist hacmi genişler. Blastokistin genişleme derecesi, hücre sayısı, iç hücre kitlesi ve ilkel trofoblast hücreleri ile uyum halinde beşinci ve altıncı günde en ileri seviyeye ulaşır.Yardımcı üreme tekniklerinde embriyoların dondurulması sıklıkla kullanılmaktadır. Embriyo dondurma çoklu embriyo transferi ihtiyacını ortadan kaldırarak çoğul gebelik oranını azaltır. Bununla beraber transfer edilmeyen veya edilemeyen embriyolarında bir sonraki süreçte kullanılmak üzere dondurulmasıda doğurganlık potansiyelinin korunması adına önemlidir.Yapılan çalışmalarda blastosöl sıvısının hızlıca azaltılması ile oluşturulan dondurmada buz kristali oluşumunun azaldığı ve embriyo canlılığının olumlu etkilendiği görülmüştür. Bunun sebebinin kriyoprotektif ajanların daha az blastosöl sıvısıyla maruziyetinden kaynaklandığı düşünülmektedir. Ayrıca blastosöl sıvısının aspirasyonu ile hücre dışı ve hücre içi dinamik denge eşitlenerek kriyoprotektif ajanlardan daha az etkilendiği düşünülmektedir.Ayrıca infertilite tedavisinde in vitro fertilizasyon (IVF) kullanım oranı %1-5 ‘dir. Ancak, bunlardan sadece küçük bir oranı sağlıklı bir gebelik için gerekli olan genetik ve metabolik gereksinimlere sahiptirler. Embriyo yeterlilik değerlendirmesi için günümüzde kullanılan morfolojik skorlama tekniği yetersizdir. Klinik embriyoloji başarı oranlarını artırmak için; embriyoların implantasyon öncesi blastosöl sıvısı analizleri; polar cisim, blastomer veya trofoektoderm biyopsilerine alernatif yeni bir yöntem olarak öne çıkmaktadır.Blastokistlerden blastosöl sıvısı aspire edilmesi için zona pellucidaya tek bir lazer darbesi TE ve IHK zarar vermeden lazerin oluşturduğu ısı etkisiyle bir delik açarak buradan rutin IVF kliniklerinde kullanılan intrastoplazmik sperm enjeksiyonu (ICSI) cihazı yardımı ile blastosentez adı verilen yöntem ile aspire edilir.
