GESTASYONEL DİYABETLİ GEBELERDE FETAL VE MATERNAL APELİN DÜZEYLERİ

Amaç: Gestasyonel diyabetes melllitus (GDM)'u olan ve olmayan gebelerde maternal ve fetal serum apelin seviyelerini değerlendirmek.Gereç ve Yöntem: Çalışmaya 52 GDM tanılı gebe ve gebelik haftası çalışma grubu ile uyumlu 30 sağlıklı gebe dahil edildi. GDM'li hastalardan kan şekeri diyetle regüle olan 27 hasta (GDM A1), insülin ihtiyacı olan 25 hasta (GDM A2) grubu olarak ayrıldı. Maternal ve fetal serum apelin seviyeleri enzim bağlı immünosorbent analizi (ELISA) yöntemiyle ölçülmüş olup, örnekler doğum sırasında alınmıştır. Bulgular: Maternal serum apelin seviyeleri gruplar arasında benzer bulundu. GDM A1 404 pg/ml , GDM A2 440 pg/ml ve kontrol 405 pg/ml , (p:0,429). Aynı şekilde fetal apelin seviyeleri de gruplar arasında benzerdi. GDM A1 468 pg/ml , GDM A2 551 pg/ml ve kontrol 461 pg/ml , (p:0,382). Maternal and Fetal Apelin Levels in Gestational Diabetes Mellitus

Objectives: To evaluate maternal and fetal serum apelin concentrations in pregnant women with and without gestational diabetes mellitus ( GDM).Material and Methods: Fifty two pregnant women with GDM and 30 gestational age matched healthy pregnant subjects participated to the study. Twenty seven patients who were regulated by blood glucose diet were separated as GDM A1 group and 25 patients who required insulin from GDM patients were assigned to the GDM A2 group. Maternal and fetal serum apelin levels were measured with Enzyme-Linked Immuno Sorbent Assay (ELISA), at the time of birth.Results: Maternal serum apelin levels were similar among the groups; GDM A1 404 pg/ml (298540), GDM A2 440 pg/ml ve control 405 pg/ml , (p:0,429). Similarly, fetal serum apelin levels were similar among the groups; GDM A1 468 pg/ml , GDM A2 551 pg/ml ve control 461 pg/ml , (p:0,382). Maternal apelin levels correlaeted positively with their respective fetal levels. Maternal and fetal serum apelin levels correlated negatively with the gestational age (maternal r:-0,448, p:0,019)(cord r:-0,375, p:0,014). In addition, maternal apelin levels correlated negatively with maternal weight and body mass index (r:-0,350/-0,386, p:0,045/0,047). We didn't find a correlation between maternal and cord serum apelin levels, maternal age, maternal height, fasting glucose, fasting insülin, birth weight, placental weight, cord blood glucose, cord blood insulin, Hemoglobin A1c (HbA1c) and Homeostatic Model Assessment-Insülin Rezistansı ( HOMA-IR) levels. Conclusion: In our study, there was no relationship between maternal and fetal apelin levels and GDM or insulin resistance indices.
