FASCİOLA HEPATİCAYA BAĞLI GELİŞEN AKUT KOLANJİT: ENDOSKOPİK RETROGRAD KOLANJİYOPANKREATOGRAFİDE SÜRPRİZ TANI Acute Cholangitis of Fasciola Hepatica: A Surprising Diagnosis in Endoscopic Retrograde Cholangiopancreatography
ÖZET
Fasciola hepatica; özellikle koyun, keçi, sığır, manda gibi hayvanları tutan; nadir olarak da insan safra yollarına yerleşerek hastalık yapan bir parazittir. Bu olgu sunumu ile kolanjit tablosu ile başvuran, endoskopik
retrograd kolanjiyopankreatografi (ERCP) ile başarılı bir şekilde tanısını koyduğumuz ve tedavisini yaptığımız,
bölgemizde çok nadir görülen bir Fasciola hepatica vakasını sunmayı amaçladık. 56 yaşında bulantı, kusma,
karın ağrısı, ateş şikayetleriyle başvuran kadın hastada klinik, laboratuvar ve görüntüleme bulguları kolanjit
ile uyumlu idi. Yapılan ERCP’ de balon ile koledok sıvazlandığında, 3 adet erişkin Fasciola hepatica paraziti
görüldü ve koledoktan çıkarıldı. Hastaya 10mg/kg/gün dozunda Triklabendazol tedavisi uygulandı. Fasciola
hepatica nadir görülmesi ve belirti-bulgularının diğer enfeksiyon hastalıkları ile benzerliği nedeniyle sıklıkla
gözden kaçabilmektedir. Karın ağrısı, ateş, bulantı-kusma kliniği ile başvuran hastalarda, ayırıcı tanıda Fasciola hepatica akılda bulundurulmalıdır.

ABSTRACT
Fasciola hepatica is a parasite that generally infects animals such as sheep, goats, cattle, buffaloes and also
rarely human biliary tract. Herein, we present a case of Fasciola hepatica, which is very rare in our region,
who presented with cholangitis. The patient was diagnosed and treated successfully with endoscopic retrograde cholangiopancreatography (ERCP). A 56-year-old female patient was presented with cholangitis by
the complaints of nausea, vomiting, abdominal pain and fever. Three adult Fasciola hepatica parasites were
detected and removed from choledochus by ERCP. The patient was administered triclabendazole at a dose
of 10mg/kg/day. Fasciola hepatica may be overlooked because of its rarity and similarity of symptoms and
signs with other infectious diseases. Fasciola hepatica should be kept in mind in the differential diagnosis of
patients presenting with abdominal pain, fever, and nausea.
