DEKOMPRESİF HEMİKRANİEKTOMİ GEREKLİ Mİ? NE ZAMAN YAPMALI?: RETROSPEKTİF ÇALIŞMA Is Decompressive Hemicraniectomy Necessary? When To Do?: Retrospective Study
ÖZET
Amaç: Akut orta serebral arter tıkanıklığına bağlı inmelerde dekompresif hemikraniektominin gerekli olup
olmadığını, yapılacak ise en uygun zamanın belirlenmesini literatür bilgileri ile karşılaştırmayı ve sunmayı
amaçladık.
Gereç ve Yöntemler: Ocak 2015- Aralık 2018 tarihleri arasında Afyonkarahisar Sağlık Bilimleri Üniversitesi
Tıp Fakültesi Hastanesi Nöroloji servisinde izlenen 84 hastadan medikal tedaviye rağmen kliniği gerileyen 22
hastaya Afyonkarahisar Sağlık Bilimleri Üniversitesi Tıp Fakültesi Hastanesi Nöroşirurji AD tarafından dekompresif
hemikraniektomi uygulandı. Hastaların tamamına geniş hemikraniektomi + duraplasti operasyonu
yapıldı.
Bulgular: Hastalar yaş, Glasgow Koma Skalası (GKS) ve dekompresyon zamanına göre retrospektif olarak
incelendi. Hastaların yaş aralığı 45-85 arası ve cerrahiye alındığı andaki GKS değerleri ortalama 9,04 (6-13
arası) idi. Cerrahiye alınma süreleri 6-240 saat arasında değişmekte idi. Cerrahi uygulanan hastalardan 16
hastanın mRS puanı beş; 6 hastanın ise dört puan idi. Toplam 22 hastadan 14 hasta sistemik sorunlar nedeniyle
kaybedildi.
Sonuç: Orta serebral arter tıkanıklığına bağlı gelişen akut inmelerde cerrahi dekompresyonun başarısını etkileyen
faktörler; hastanın yaşı, cerrahi öncesi glaskow koma skoru ve semptomlar başladıktan sonra cerrahiye
alınma süresidir. Erken dönem yapılan cerrahinin sağ kalım üzerine olumlu etkileri vardır.

ABSTRACT
Objective: We aimed to compare and present whether or not decompressive hemicraniectomy is necessary
in strokes due to acute middle cerebral artery occlusion, and to determine the best time to be performed
with the literature.
Material and Methods: From January 2015 to December 2018, 22 patients who had a clinical regression
despite the medical treatment from Afyonkarahisar Health Sciences University Medical Faculty Hospital
Neurology service underwent decompressive hemicranectomy by Afyonkarahisar Health Sciences University
Medical Faculty Hospital Neurosurgery AD. All patients underwent extensive hemicraniectomy + duraplasty
operation.
Results: The patients were retrospectively analyzed according to age, Glaskow Coma Scale (GKS) and decompression
time. The age range of the patients was between 45-85 and the mean GKS at the time of
surgery was 9.04 (between 6-13). The duration of the surgery ranged from 6 to 240 hours. Of the patients
undergoing surgery, the mRS score of 16 patients was five; Six patients were four points. Of the 22 patients,
14 were lost due to systemic problems.
Conclusion: Factors affecting the success of surgical decompression in acute strokes due to middle cerebral
artery occlusion; the age of the patient, the glaskow coma score before surgery, and the duration of surgery
after symptoms begin. Early surgery has positive effects on survival.
