BOZUCU ALAN VE MYOFASİAL TETİK NOKTA KAYNAKLI KRONİK AĞRI ve PANİK ATAK HASTASINA NÖRALTERAPİ YAKLAŞIMI

Bu olguda her 2 alt ekstremitede bacak ve baldır bölgesinde, sol kardiyak bölgede ve baş-boyun bölgesinde ağrı şikayetleri bulunan ve panik atak tanısı konulmuş hastaya uygulanan bütünsel yaklaşım ve nöralterapi uygulamaları değerlendirilmiştir. Anamnezinde 4 yıl önce her 2 alt ekstremitede batma ve yanma tarzında ağrı başladığını ve bu olaydan yaklaşık 1 yıl sonra gece ani göğüs ağrısı kalpte sıkışma hissi, nefes alamama, baş boyun bölgesine yayılan ağrı ile birçok sefer acile başvurduğunu ifade etmiştir. Bu tabloya baş dönmesi, ayaklarda yanma üşüme, uyku bozukluğu ve sürekli yorgunluk hissi de eklenmiş. Kardiyoloji, göğüs hastalıkları ve fizik tedavi bölümlerinde değerlendirilen hastaya duloksetin 30 mg, gabapentin 600 mg 3x1 dozunda başlanmış. Hastanın zamansal sorgulamasında 2010 yılında diş tedavisi olduğunu ve iş değişikliği yaşadığını belirtmiştir. Sık hastalık, şişkinlik ve zaman zaman kabızlık şikayetleri olduğunu ifade etmiştir. Yapılan muayene bulguları sonucunda hastaya; Hormonal disfonksiyon, barsak flora bozukluğu, latent asidoz, torakal blokaj, bozucu alan ve myofasial ağrı tanıları konulmuştur. Hastada bozucu alanların ortaya çıkardığı tablo segmental ve segment üstü tedavilerle kontrol altına alınmıştır. Yardımcı solunum kaslarında gelişen tetik noktaların ağrı yayılımları, panik atak ve nefes alamama hissine neden olarak kardiyolojik problemi taklit etmiştir. İlgili kaslara tetik nokta enjeksiyonları germe ve segmental/segmentüstü tedavilerle çözülmüştür. Yapılan ganglion enjeksiyonları ile ağrının kalıcı hale gelmesi engellenmiş ve bozucu alan enjeksiyonları ile de regülasyon sağlanmıştır.

NEURALTHERAPY APPROACH FOR CHRONIC PAIN and PANIC ATTACK DISORDER PATIENT WITH INTERFERENCE FIELD and MYOFACIAL TRIGGER POINT INJECTIONS

In this case we evaluated the results of neuraltherapy and holistic approach to a patient who has pain in bilateral lower limb, left cardiac zone and head neck as well. Also the patient was diagnosed as panic disorder.In his anamnesis; pain begins suddenly before 4 years ago bilaterally in his lower limbs with a burning sensation. He stated that ; he was admitted to the emergency service several times with a sharp, sudden onset chest pain, shortness of breath , and a pain that is radiating from neck to head region 1 year after the beginning of his pain. Dizziness, burning and chilling feet sensations, sleep disturbance, and the feeling of constant fatigue added to the patients complaints. Evaluations of the patient was made in the Cardiology, Pulmonology and Physical Therapy and Rehabilitation departments he was started duloxetine 30 mg and 600 mg of gabapentin in 3x1 dose. In temporally questioning of the patient, he stated that, he had a dental treatment in 2010 and a change of job in the same year as well. He also has frequent illness, bloating and constipation complaints from time to time. In physical examination findings of the patient ; hormonal dysfunction, intestinal flora disorder, latent acidosis , thoracic blockade , interference field and myofascial pain were diagnosed .The complaints of the patient that arises from interference fields, was brought under control with segmentally and suprasegmentally neuraltherapy injections. Radiating pain from trigger points in the accessory respiratory muscles are mimicked the cardiologicalproblem by causing the sensation of panic attacks and inability to breathe. After trigger point injections and post isometric relaxation of the muscles, related segmentally and suprasegmentally neuraltherapy injections was solved the problem. With the ganglion injections, the pain becomes permanently blocked and interference field injections provided the regulation of the patient
