GEBELİKTE ÜRİNER SİSTEM İNFEKSİYONLARININ AMPİRİK TEDAVİSİNDE HANGİ ANTİBİYOTİKLER İLK SEÇENEK OLMALIDIR?
Üriner sistem infeksiyonu gebelikte sık görülen bir sağlık sorunudur. Ancak, gebelikte güvenle kullanılabilen antibiyotikler fetal toksisite riski nedeniyle kısıtlıdır. Bu çalışmada gebe kadınların idrar kültürlerinden izole edilen patojenlerin antimikrobiyal direnç profillerinin belirlenmesi ve ampirik tedavide tercih edilebilecek antibiyotiklerin tespiti amaçlanmıştır. Ocak 2015 - Ocak 2016 tarihleri arasında Karabük Eğitim ve Araştırma Hastanesi’ne başvuran gebelerin idrar örneklerinden izole edilen bakterilerin identifikasyonu ve antibiyotik duyarlılıkları BD-Phoenix (Becton-Dickinson, USA) tam otomatize sistem ile, genişlemiş spektrumlu beta-laktamaz varlığı (GSBL) ise kombine disk difüzyon yöntemi ile araştırılmıştır. Sonuçlar retrospektif olarak incelenmiştir. Toplam 2,697 idrar örneğinin 275 tanesinde (% 10) anlamlı bakteri üremesi (105 cfu/ml ve üzeri) gözlenmiştir. Üreyen bakterilerin 223’ü Gram negatif (% 81), 52’si (% 19) Gram pozitiftir. En sık izole edilen patojen Escherichia coli olup (% 67) bunu sırasıyla Streptococcus agalactiae (% 11) Klebsiella pneumoniae (% 9) ve Enterococcus faecalis (% 5) izlemiştir. Gebelikte güvenle kullanılabilen oral antibiyotiklerden ampisilin ve amoksisilin-klavulanik asit direnci E.coli suşlarında sırasıyla % 53 ve % 26 idi. E.coli suşlarında sefuroksim ve sefiksim direnci sırasıyla % 18 ve % 10, K.pneumoniae suşlarında ise % 12 olarak tespit edilmiştir. Fosfomisin direnci ise sırasıyla % 3 ve % 17 olarak belirlenmiştir. İn vitro en etkili oral antibiyotik nitrofurantoin idi. GSBL oranı E.coli suşlarında % 8 ve K.pneumoniae suşlarında % 13 olarak tespit edilmiştir. Bu çalışmanın sonuçları, gebelikte üriner enfeksiyonların ampirik tedavisinde yüksek direnç nedeniyle ampisilin ve amoksisilin-klavulanik asitten kaçınılması gerektiğini göstermiştir. Onun yerine, sefuroksim, sefiksim, fosfomisin ve nitrofurantoin oral alınabilmeleri ve düşük direnç oranları nedeniyle tercih edilebilir.
Which Antibiotics Should be First Line Options for Empirical Treatment of Urinary Tract Infections During Pregnancy?
Urinary tract infection is a common health problem during pregnancy. However, antibiotics that can be safely used in pregnancy are limited due to the risk of fetal toxicity. This study aimed to identify the antimicrobial resistance profiles of pathogens that were isolated from urine cultures of pregnant women and determine the antibiotics that can be preferred in empirical treatment. The identification and antibiotic susceptibility of the bacteria isolated from urine samples of the pregnant women that visited to Karabük Education and Training Hospital between January 2015 and January 2016 were performed using a BD-Phoenix (BectonDickinson, USA) fully automated system. Also, the presence of extended-spectrum beta-lactamase (ESBL) was investigated by the combined disk diffusion method. The results were analyzed retrospectively. A significant bacterial growth (105 cfu/ml and above) was observed in 275 (10 %) of 2,697 urine samples. Of these bacteria, 223 (81 %) were Gram negative, and 52 (19 %) were Gram positive. The most frequently isolated pathogen was Escherichia coli (67 %), followed by Streptococcus agalactiae (11 %), Klebsiella pneumoniae (9 %), and Enterococcus faecalis (5 %). The resistance against ampicillin and amoxicillin-clavulanic acid which are antimicrobials that can be safely used during pregnancy, was 53 % and 26 % respectively. The resistance of cefuroxime and cefixime in E.coli strains was 18 % and 10 % respectively, and 12% in K.pneumoniae strains. Fosfomycin resistance rates of E.coli and K.pneumoniae strains were 3% and 17 % respectively. Nitrofurantoin was the most effective oral antibiotic in-vitro. ESBL positivity was 8 % in E.coli strains and 13 % in K.pneumoniae strains. The results of this study indicated that ampicillin and amoxicillin-clavulanic acid should be avoided for empirical treatment of urinary tract infections during pregnancy due to high resistance rates. Instead, cefuroxime, cefixime, fosfomycin and nitrofurantoin can be preferred due to their oral availability and low resistance rates
