Antifosfolipid Sendromlu Hastaların Klinik Özellikleri ve Tekrarlayan Trombozu Olan Hastaların Farklılıkları, Tek Merkezli Retrospektif Bir Çalışma

Amaç: Bu çalışma üçüncü basamak sağlık hizmeti veren bir merkezde takip edilen antifosfolipid sendromu (AFAS) tanılı hastaların klinik, laboratuvar ve serolojik özelliklerini tanımlamayı ve tekrarlayan trombozu olan hastaların olası farklılıklarını ortaya koymayı amaçlamıştır. Gereç ve Yöntem: Ocak 2010-Ocak 2019 tarihleri arasında takip edilen 43 AFAS hastası geriye dönük olarak incelendi. Hastaların klinik, laboratuvar ve serolojik özellikleri kayıt altına alındı. Takibinde tekrarlayan trombozu olan ve olmayan hastalar karşılaştırıldı. P değerinin 0,05’in altında olduğu durumlar istatistiksel olarak anlamlı kabul edildi. Bulgular: Kırk üç hastanın 33’ü (%76,6) kadındı. On dört (%32,6) hasta primer AFAS, 29 (%67,4) hasta ise sekonder AFAS olarak sınıflandırıldı. AFAS sınıflandırma kriterlerinde olmayan en sık görülen klinik bulgular arasında akut kutanöz lupus bulguları (%53,5), livedo retikülaris (%18,6), raynaud fenomeni (%18,6) ve trombositopeni (%30,2) ön plana çıkmaktaydı. Kadın hastaların %45,5’inde gebelik komplikasyonu tespit edildi. Kırk (%93) hastada tromboz öyküsü mevcuttu. Trombozların %76,7’si venöz %32,6’sı arteryel trombozdu. Antikoagülan tedavi altında olan 9 (20,9) hasta da dahil olmak üzere toplam 20 hastada (%50) tekrarlayan tromboz olduğu saptandı. Tekrarlayan trombozu olan hastalarda hastalık süresi daha uzun iken (p=0,004), arteryel tromboz varlığı (p=0,023) ve düşük serum C4 düzeyleri (p=0,025) daha sıktı. Sonuç: Bu çalışmada tekrarlayan tromboz ile takipli AFAS hastalarında hastalık sürelerinin daha uzun, arteryel tromboz öyküsünün daha fazla ve düşük serum C4 düzey varlığının daha sık olduğu gösterilmiştir.

Clinical Features of Patients with Antiphospholipid Syndrome and Differences of Patients with Recurrent Thrombosis, A Single Center Retrospective Study

Objectives: The study aimed to reveal the clinical, laboratory and serological features of patients with antiphospholipid syndrome (APS) followed in a referral center and to demonstrate the possible differences of the patients with recurrent thrombosis. Materials and Methods: The data of 43 patients with APS, who applied to our center between January 2010 and January 2019, were scrutinized retrospectively.
