Alt dudak defektlerinin rekonstrüksiyonu: 43 olgunun değerlendirilmesi
Dudaklar, sadece estetik açıdan değil aynı zamanda ifade, mimikler, yemek yeme ve konuşma açısından da önemli yapılardır. Bu yüzden hasarlı kısımların onarımı, hem estetik hem de fonksiyonel açıdan büyük önem taşımaktadır. Dudak onarımlarının amacı, görünümün ve fonksiyonun yeniden kazandırılmasıdır. Bu amaçla değişik rekonstrüksiyon yöntemleri kullanılmaktadır. Dudak defektinin cerrahi planından önce bazı değişken durumların önceden tesbit edilmesi gerekmektedir: Defektin boyutu ve lokalizasyonu, lezyonun etiyolojisi, hastanın yaşı ve cinsiyeti gibi. Hastalarda cilt, kas, mukoza ve bunların kombinasyonu gibi değişik derecelerde doku kaybı bulunabilir. Kayıp, travma sonucunda olabileceği gibi, enfeksiyöz hastalıklar, vaskülit, konjenital nevüs, hemanjiom, yarık ve neoplazmalara bağlı olarak olarak da gelişebilir. Rekonstrükte edilmiş dudak; duyusu olan, sfinkter veya kas fonksiyonunu devam ettiren, alt ve üst vermilyonlar arasında su kaçırmaz özellikte olan, yemek ve dental bakım için yeterli ağız açıklığı sağlayan ve kabul edilebilir bir estetik görünüme sahip olmalıdır. Önemli olan cerrahın uygun yöntemi doğru vakalarda uygulamasıdır. Bu makalede; "V" rezeksiyon, "W" rezeksiyon, "Barrel shaped" eksizyon, "Stair Step" eksizyon, "Abbe" flep, "Webster Bernard" flep, "Estlander" flep, "Gilles" fan flep, "Karapandzic" flep, "Nakajima" flep, "Depressoör Anguli Oris" flepleri kullanılarak gerçekleştirdiğimiz kırküç olgu ve. sonuçları sunulmuştur.
Reconstruction of lower up defects: Analysis of 43 cases
Lips are important structures not only of aesthetic value but also for expression, vitality, feeding and speaking. As such, both functional and aesthetic restoration of deformed parts receive high priority. The aims of reconstruction of the lips are the restoration of appereance and reinstitution of function. To achieve these results, different reconstructive methods are used. Prior to formulation of a surgical plan for a lip defect, the following variables are evaluated: Size and location of the defect, etiology of the lesion, and the age and sex of the patient. Patients present with varying degrees of tissue loss, which may include skin, muscle, or mucosa, or a combination of layers. Loss may be a result of trauma, infectious disease, vasculitis, congenital nevi, hemangiomas, clefts, or neoplasm. The reconstructed lip should be sensate, maintain sphincter or muscle function, and allow apposition of lower vermilion to upper vermilion for a watertight seal, provide sufficient opening for food and dental care, and be of acceptable aesthetic appearance. The important part is, surgeon must apply the appropriate method to the correct cases. In this paper fortythree patients, who were treated with "V" resection, "W" resection, "Barrel shaped" excisions, "Stair Step" excisions, "Abbe" flap, "Webster Bernard" flap, "Estlander" flap, "Gilles" fan flap, "Karapandzic" flap, "Nakajima" flap, "Depressor Anguli Oris" flap, and their results are presented.
