Açık kalp cerrahisi sonrası erken ekstübasyon; rutin işlem olabilir mi?

Son yıllarda açık kalp cerrahisinde tercih edilmeye başlayan erken ekstubasyonun rutin işlem olabilirliğini göstermek için prospektif olarak bu çalışma yapılmıştır. Çalışmaya iki ayrı klinikte 2 yıl boyunca 375 çalışma grubu, 382 kontrol grubu olmak üzere toplam 757 hasta dahil edilmiştir. Tüm hastaların demografik verileri, intra-operatif verileri, postoperatif komplikasyonlar; entubasyon süreleri, yoğun bakım kalış süreleri kaydedilmiştir. Çalışma grubunun yaşları 55.3 ± 11.8 yıl iken, kontrol grubununki 56.7 ± 11.5 yıldır. Ekstubasyon süreleri çalışma grubunda 4.0 ± 1.5 saat iken, kontrol grubunda 11.2 ± 8.4 saattir. Yoğun bakımda kalış süreleri ise çalışma grubunda 9.7 ± 6.1 saat iken, kontrol grubunda 20.7 ± 15.6 saattir. Erken ekstubasyon uygun anestezi protokolleriyle açık kalp cerrahisinde rahatlıkla uygulanabilir. Bu yaklaşımlar maliyet açısından da önemli faydalar sağlamaktadır.

Early extubation after open heart surgery; can it be routine procedure?

This study is performed prospectively to show the possibility of beginning to prefer early extubation as a routine procedure in open heart surgery recently. In this study, 375 study group, 382 control group, 757 overall patients were included for two years in two different clinics. All patients demographics, intraoperative data, postoperative complications, entubation time and intensive care unit periods were recorded. In study group age was 55.3 ± 11.8 years, in control group age was 56.7 ± 11.5 years. Extubation time in study group was 4.0 ± 1.5 hours, in control group was 11.2 ± 8.4 hours. Intensive care unit period in study group was 9.7 ± 6.1 hours, in control group was 20.7 ± 15.6 hours. Early extubation with preferred anestesia protocols can be performed readily in open heart surgery. This approach also provides significant benefits in cost.
