YOĞUN BAKIM TAKİBİ GEREKTİREN OBSTETRİK DURUMLAR: ANKARA EĞİTİM VE ARAŞTIRMA HASTANESİ DENEYİMİ
AMAÇ: Hastanemiz kadın hastalıkları ve doğum kliniğinde tedavigören ve yoğun bakım takibi gerektiren obstetrik durumları özet- lemekMATERYAL-METOT: Kliniğimizde 2015-2018 yılları arasındayatan gebeler retrospektif olarak incelendi. Bu dönemde yoğun ba- kımda tedavi edilen 34 hastanın bilgileri elektronik data ve hastadosyaları incelenerek özetlendi.BULGULAR: Ocak 2015-Aralık 2017 arasında kliniğimizde 9110doğum gerçekleşti. Yatarak tedavi edilen hastaların 34 tanesinde yo- ğun bakım ihtiyacı gelişti (%0,37). En sık yoğun bakıma yatış endi- kasyonları hipertansiyon (n=21, %61,7) ve kanama idi (n=6, %11,8).Gelişen tek mortalite plasenta dekolmanına bağlı postpartum ka- nama kaynaklıydı (n=1, %2,9). Yoğun bakımda tedavi edilen has- talarımızda APACHE II skorlaması ile öngörülen ölüm %9,9 ikengerçekleşen ölüm oranı ise %2,9 idi.SONUÇ: Kliniğimizde obstetrik vakalar için yoğun bakıma yatmaoranı 1000 doğum başına 3,7’dir. Hem yoğun bakıma yatış sebeplerihem de oranları gelişmiş ülkelere benzerlik göstermektedir. Tüm ül- kemizin durumunu yansıtan daha geniş çalışmalara da gereksinim vardır.
OBSTETRIC PROBLEMS REQUIRING INTENSIVE CARE UNIT ADMISSION: ANKARA TRAINING AND RESEARCH HOSPITAL EXPERIENCE
OBJECTİVE: To summarize obstetric conditions that require in- tensive care in our hospital. MATERİALS AND METHODS: The data of obstetric patients tre- ated in our clinic between 2015-2018 was reviewed retrospectively. In this period, the information of 34 patients who were treated in intensive care unit was summarized by reviewing electronic data and patient files. RESULTS: Between January 2015 and December 2017, there were 9110 deliveries took place in our clinic. There were 34 patients who needed of intensive care admission (0.37%). The most frequent indi- cations for intensive care unit admission were hypertension (n = 21, 61.7%) and obstetric hemorrhage (n = 6, 11.8%). One patient died due to postpartum hemorrhage. In this patient, bleeding was due to placental abruption. (n = 1, 2,9%). The expected mortality rate calculated from the APACHE II score was 9.9% while the mortality rate was 2.9%. CONCLUSİON: In our clinic, the rate of intensive care for obstetric cases is 3,7 per 1,000 births. Both causes and rates of intensive care admissions of obstetric patients are similar to developed countries. However, there is also a need for larger studies that reflect the situation of all Turkey.
