ANKİLOZAN SPONDİLİTLİ HASTALARDA KİNEZYOFOBİNİN HASTALIK AKTİVİTESİ ve FONKSİYONEL DURUMA ETKİSİ

Amaç Çalışmamızın amacı ankilozan spondilit (AS)’li hastalarda kinezyofobi düzeyinin hastalık aktivitesi, fonksiyonel durum, depresyon, yorgunluk ve yaşam kalitesi üzerine olan etkisini değerlendirmektir. Gereç ve Yöntemler Çalışmaya 46 (31 erkek, 15 kadın) AS’li hasta ile 40 (11 erkek, 29 kadın) sağlıklı kontrol dahil edildi. Yaşam kalitesi Sağlık Değerlendirme Anketi (SDA), hastalık aktvitesi Bath Ankilozan Spondilit Hastalık Aktivite İndexi (BASDAI), fonksiyonel durum Bath Ankilozan Spondilit Fonksiyonel İndex (BASFI), ağrı şiddeti Vizüel Analog Skala (VAS), kinezyofobi Tampa Kinezyofobi Skalası (TKS), yorgunluk VAS, depresyon Beck Depresyon Envanteri (BDE) ve fiziksel aktivite düzeyi Uluslararası Fiziksel Aktivite Anketi (UAFAA) kısa form ile değerlendirildi Bulgular TKS (p <0,001), VAS-ağrı (p:0,000), VAS-yorgunluk (p:0,037), SDA (p:0,000), BDE (p:0,002), BASDAI (p:0,000) ve BASFI (p:0,000) skorları AS grubunda sağlıklı kontrollere göre daha yüksekken, fiziksel aktivite düzeylerinde (p:0,412) istatistiksel açıdan anlamlı fark yoktu. AS'li hastalarda TKS skoru ile VAS-ağrı (r:0,684, p:0,000), VAS-yorgunluk (r:0,494, p:0,000), BDE (r:0,321, p:0,033), BASDAI (r:0,484, p:0,001) ve BASFI (r:0,389, p:0,008) skorları ilişkili bulundu. Sonuç AS’li hastalarda kinezyofobi oranları daha yüksek olup; hastalık aktivitesi, fonksiyonel durum, ağrı, yorgunluk ve depresyonla ilişkili bulunmuştur. Katastrofik düşünce ve hareket korkusu eğilimi olan AS’li hastaları önceden tespit edip, kinezyofobiyi tedavi edebilirsek AS’li hastaların nonfarmakolojik tedavisinde önemli bir ilerleme kaydedebileceğimizi düşünmekteyiz.
