4 mm’lik testis tümörüne ultrason eşliğinde işaretleme ile organ koruyucu cerrahi

Testis kanseri 15–45 yaş arası erkeklerde en sık görülen malignitedir. Ultrasonun yaygın kullanımıyla birlikte küçük asemptomatik testisküler kitlelerin tanısında da artış olmuştur. Radikal orşiektomi testis kanserinin standart tedavisi olmasına karşın; son yıllarda bilateral testis kanseri ve küçük testiküler kitlelerin tedavisinde umut verici onkolojik ve fonksiyonel sonuçları nedeniyle testis koruyucu cerrahinin yeri artmıştır. 2 cm’den küçük testis kitlelerinde uygun hastalara testis koruyucu tedavi yapılabilmesi önerilmektedir. Fakat kitlenin küçük olması cerrahi sırasında bulunabilirliği ve eksizyonu zorlaştırmaktadır. Literatürde özellikle küçük testis tümörleri için organ koruyucu tekniği gösteren az sayıda yayın vardır. Bizde 4 mm çaplı oldukça küçük testis tümörü olan hasta da yaptığımız parsiyel orşiektomi olgusunu ve kullandığımız tekniği paylaşmak istedik.

Organ preserving surgery with ultrasound-guided marking in 4 mm testicular tumor

Testicular cancer is the most common malignity seen in males between the ages of 15 and 45. There has been an increase in the diagnosis of small asymptomatic testicular masses with extensive ultrasound use. Although radical orchiectomy is the standard treatment for testicular cancer, the importance of testicle preserving surgery has increased in recent years due to its promising oncological and functional results in the treatment of bilateral testicle cancer and small testicular masses. Testicle preserving treatment is suggested for testicular masses smaller than 2 cm in suitable patients. But small mass size makes the detectability and excision harder during the surgery. In literature, there are limited publications demonstrating organ preserving technique especially for small testicular tumors. Our aim was to share the partial orchiectomy case which had a quite small testicular tumor with a diameter of 4 mm and the technique we used for this case.
