Ketoasidoz Tablosuyla Prezente Olan Yeni Tanı Diabetes Mellitus ve COVID-19 Birlikteliği: Olgu Sunumu.

SARS-CoV-2 virüsünün spike proteini yardımıyla tutunduğu fonksiyonel anjiyotensin dönüştürücü enzim 2 (ACE2) reseptörleri pankreas endokrin adacıklarda yoğun miktarda bulunur ve SARS-CoV-2 için giriş noktası olarak işlev görür. SARS-CoV-2’nin pankreatik adacık hücrelerine girişi ile direkt olarak beta hücre hasarı yapması ve virüsün hücre içine girişini takiben ACE2’nin ‘down’-regülasyonu sonucu artan anijotensin-2’nin insülin sekresyonunu azaltması olduğu düşünülmektedir. Bu olguda diyabet tanısı ve steroid kullanımı olmayan, ketoasidoz tablosuyla başvuran yeni tanı diabetes mellitus ve COVID-19 birlikteliği sunulmuştur.

The Association of Newly Diagnosed Diabetes Mellitus and COVID-19 Presenting with Ketoacidosis: A Case Report

Angiotensin converting enzyme 2 (ACE2) receptors, to which the SARS-CoV-2 virus attaches with the spike protein, are abundant in pancreatic endocrine islets and act as the entry point for SARS-CoV-2. It is thought that SARS-CoV-2 causes beta cell damage directly with its entry into pancreatic islet cells and angiotensin-2, which increases as a result of down-regulation of ACE2 following the entry of the virus into the cell, reduces insulin secretion. In this article, the association of newly diagnosed diabetes mellitus and COVID-19 presenting with ketoacidosis, without previous diagnosis of diabetes and steroid use, is presented.
