Torakotomi sonrası iyatrojenik trake-osafegeal fistül nedenli solunum yetmezliği (Olgu sunuımu)
Sarkoidoz ön tanısı nedeniyle torakotomi yapılarak mediastinal lenf bezi biyopsisi alınması planlanan 10 yaşında kız çocuğu, operasyon bittikten sonra ekstübasyon sonrası solunum sıkıntısı gelişmesi ve oksijen satürasyonun düşmesi üzerine alınan kan gazında; Ph: 7,51, pCO2: 44mmHg, PO2: 47 mmHg, HC03: 34,3mmol/l, BE: +10.3, SpO2: % 86 saptanması üzerine entübe edilerek yoğun bakıma alındı. TV: 5ml/kg, f: 15/dk, PEEP: 5cmH2O, FİO2: %50, PSV: 15 mbar, değerleri ile senkronice inspiratuvar zorunlu ventilasyon(SIMV) modunda mekanik ventilator e bağlandı. Yoğun bakıma alındıktan sonra 6. saatte batın distansiyonu ve nazogastrik kateter torbasında hava birikimi gelişmesi üzerine iyatrojenik TEF geliştiği düşünüldü ve total parenteral beslenmeye başlandı. Yatışının 2 .günü fleksible bronkoskopi yapıldı. Ancak fistül ağzı görülemedi.Şüpheli TEF, sekonder iyileşmeye bırakıldı. Endotrakeal tüpün yeri mide dilatasyonunu arttırmayan bir noktada tespit edildi. Barotravma riskini azaltmak için basınç kontrollü (PC) moda geçildi. Biyopsi sonucunun kesin sarkoidoz gelmesi üzerine prednol (2x 25mg) İV başlandı. Rutin tetkiklerin dışında, AC grafi, sedimantasyon, CRP, ALT, AST günlük olarak takip edildi.Yatışının 19.günü hasta ekstübe edildi. 3 gün boyunca enteral beslenmeyi tolere etmesi ve oda havasında alınan kan gazı örneğinde; pH: 7,4, pCO2:42mmHg, PO2:62 mmHg, HCO3:26 meqll, BE: +1,2, SpO2: %95 olması üzerine ilgili servise devredildi. Sonuç olarak; torakotomi yöntemiyle mediastinal lenf bezi biyopsisi alınırken iyatrojenik olarak gelişen, ve nadir bir komplikasyon olan TEF' in tedavisinde; endotrakeal tüpün, fistül seviyesinin distaline yerleştirilmesi, aşırı volüm travmasından sakınmak için basınç kontrollü modlar uygulanması ve enteral beslenme durdurulup geçici bir süre parenteral beslenme uygulanmasının iyileşme sürecini hızlandırdığı kanısına varıldı.
Respiratory failure due to iatrojenic tracheoesophageal fistula after thoracotomy (Case report)
A ten years old ASA H girl, previously diagnosed as sarcoidosis and planned to obtain mediastinal lenf biopsy material via thoracotomy. After surgery, arterial blood gases value in room air was; pH: 7,51, pCO2: 44mmHg, pO2: 47 mmHg, HCO3: 34,3mmol/l, BE: +10.3, SpO2: % 86. Therefore the patient was admitted to 1CU, then connected to the mechanical ventilation in SIMV mode, as Tv: SmllkgJ: 15/dk, PEEP: 5cmH20, FİO2: %50, PSV: 15 mbar. On the six th hour of admission, gastric dilatation and regurgitation symptoms has occured. So, it was thought that a tracheo-oesaphageal fistula to be developed. Parenteral feeding was started. Pressure support volume mode preferred for mechanical ventilation. The fistula has left to secondary recovery. For sarcoidosis treatment; prednisolone (2x25mg daily was administered, by iv. AC graphy, sedimentation, CRP, AST, ALT values were evaluated on daily basis. At the 19th day of admission, patient was extubated. Enteral feeding tolerated well and in room air ventilation arterial blood gases value were measured as pH: 7,4, pCO2:42mmHg, PO2: 62 mmHg, HCO3:26 meqll, BE: 1,2, SpO2: % 95, sent to related clinic. We concluded that; placing the endotracheal tube on distal level of fistula, preferring pressure control mode as a ventilation mode and to prefer parenteral nutrition would shorten the recovery period in management of TEF.
