Mandibulada Kist Benzeri Geniş Periapikal Lezyonun Cerrahi Olmayan Endodontik Tedavisi

Pulpa dokusunun; çürük, dental işlemler veya travma gibi nedenlerle enfekte olması öncelikle pulpa sağlığının yitirilmesine sonrasında ise pulpa nekrozuna neden olur. Nekroz sonrası oluşan mediatörler periapikal bölgedeki immün cevabı tetikleyerek periapikal lezyon gelişmesine neden olabilir. Kist benzeri geniş periapikal lezyonlu dişlerin tedavi seçenekleri endodontik tedavi, endodontik tedavi ile birlikte cerrahi tedavi ya da dişin çekimidir. Geçmişte, büyük periapikal lezyonlar genellikle lezyonlu dişin veya dişlerin kök kanal tedavisiyle birlikte kistik lezyonun da cerrahi olarak çıkartılması ile tedavi ediliyordu. Ancak son yıllarda, kök kanal sistemlerinin morfolojik farklılıkları hakkında daha fazla bilgi sahibi olunması ve endodonti alanındaki gelişmeler sonucunda kist benzeri büyük lezyonlu dişlerin tedavisinde artık daha az oranda cerrahi tedaviye ihtiyaç duyulmaktadır. Bu vaka raporunda; sol alt mandibular bölgesinde kist benzeri geniş periapikal lezyonu bulunan hastanın cerrahi olmayan endodontik tedavisi sonucunda sağlanan başarılı iyileşme sunulmaktadır.

Non-Surgical Endodontic Treatment of Large Cyst-Like Periapical Lesion in the Mandibula

The infection of pulp tissue because of caries, dental procedures or trauma firstly causes loss of pulp health and pulp necrosis then. The mediators formed after necrosis may trigger the immune response in the periapical region and cause the development of periapical lesions. Treatment options for teeth with cyst-like large periapical lesions are endodontic treatment, surgical treatment combined with endodontic treatment, or tooth extraction. In the past, large periapical lesions were usually treated by surgical removal of the cystic lesion along with root canal treatment. However, in recent years, as a result of more knowledge about the morphological differences of root canal systems and developments in the field of endodontics, surgical treatment is needed at a lesser rate in the treatment of teeth with cyst-like large lesions nowadays. In this case report; the successful management of a patient with a large cyst-like periapical lesion in the left lower mandibular region as a result of non-surgical endodontic treatment is presented.
