ALT EKSTREMİTE AMPUTE REHABİLİTASYONUNDA EV PROGRAMININ ÖNEMİ
Çalışma alt ekstremite ampute rehabilitasyonunda ev programının önemini göstermek amacıyla yapılmıştır. Çalışmaya Protez-Biomekanik Ünitesi’ne protez yaptırmak üzere başvuran ve preprostetik dönemde olan 60 alt ekstremite amputesi dâhil edilmiştir. Olgular iki eşit gruba ayrılmıştır. I.gruba fizyoterapist denetiminde 10 gün süreyle egzersiz programı verilmiş, daha sonra bu programa ev programı şeklinde haftalık kontrollerle devam edilmiştir. II. gruptaki hastalara, Ankara dışında oturmaları nedeniyle yalnızcaev programı verilmiş ve 2 ay sonunda tekrar değerlendirme kapsamına alınmıştır. Her iki gruptaki amputeler, ampütasyon nedeni, ampütasyon seviyesi, güdük şekli, güdük boyu, fantom hissi ve ağrısı, normal eklem hareketleri, kas kısalıkları, kas kuvveti, güdük şekli, ödem, eğitim süresi ve motivasyon açısından değerlendirilmişlerdir. Çalışmanın sonucunda, her iki grubu rehabilitasyon açısından karşılaştırdığımızda, ikinci grupta yaygın ödem, eklem hareketlerinde limitasyon, kas kuvvet yetersizliği, kas kısalıkları, güdük şekil bozuklukları ve motivasyonda yetersizlik gözlenmiştir. Egzersiz programı ve ev programının kontrollü ve düzenli yapıldığı birinci grupta bu şikâyetlerin hiçbirinin gözlenmediği ve güdük-soket-protez uyumunun iyi olduğu, eğitim süresinin kısaldığı görülmüştür. Çalışmanın sonucunda düzenli ve kontrollü yapılan bir ev programının ampute rehabilitasyonu açısından önemli olduğu görüşüne varılmıştır

IMPORTANCE OF HOME TREATMENT PROGRAMME IN LOWER LIMB AMPUTEE REHABILITATION
The aim of this study was to show importance of home treatment programme in lower limb amputee rehabilitation. Sixty lower limb amputees who applied to Prosthetics- Biomechanics Department for prosthetic fitting were evaluated. The study was carried out on two groups. An exercise programme was given to the first group by a physiotherapist for 10 days and further controls were done once a week since the patients forming the second group were living outside Ankara, home treatment programme was designed for them and called back for controls two months later. All the patients were assessed for their amputation cause, reason and level, stump length and shape, normal joint motion, muscle strength and shortening, motivation, edema and treatment period. Finally, when the two groups were compared for the outcome of rehabilitation, insufficiency in ambulation and motivation was presented in second group besides severe edema, poor muscle strength and limitations in joint motion. It was determined that the first group didn’t show such problems and presented good stump-socket fitting and successful rehabilitation through a shorter treatment period. As a result of the study, regular and controlled home treatment programme is very important in lower limb amputee rehabilitation
