Nöropsikiyatrik bulgularla başvuran nörobruselloz olgusu Olgu Sunumu
Sinir sistemi tutulumu brusellozun nadir bir komplikasyonu olup nörolojik belirtilerle seyredebildiği gibi psikiyatrik belirtilerle de seyredebilir Nörolojik olarak menenjit meningoansefalit periferik nöropati ve radikülopatiye rastlanır Psikiyatrik bulgular ise konfüzyon halüsinasyon manik ve paranoid davranışlar depresyon ve psikotik tepki gibi değişik şekillerde olabilir Çocuklarda nörobruselloz daha çok menenjit ve menengoansefalit olarak ortaya çıkmakta olup psikiyatrik bulgular nadirdir Bu olgu bildirisinde nöropsikiyatrik bulguların ön planda olduğu nörobruselloz tanısı alan sekiz yaşındaki kız hasta sunulmuştur Endemik bölgelerde açıklanamayan nöropsikiyatrik belirtileri bulunan hastalarda nörobruselloz tanısı akılda bulundurulmalıdır Türk Ped Arş 2009; 44: 103 5 Anahtar kelimeler: Bruselloz çocukluk çağı nörobruselloz nöropsikiyatrik bulgu
Neurobrucellosis in a child presenting with neuropsychiatric manifestation Case Report
Nervous system involvement is an uncommon complication of brucellosis In adults clinical manifestation may range from severe meningoencephalitis or peripheral neuropathy radiculopathy to such behavioural disturbances as depression mental inattention hallucination amnesia psychosis agitation nightmares personality disorder euphoria nervousness In contrast neurobrucellosis is usually of acute presentation in children involving the central nervous system and neuropsychiatric manifestation is rarely observed The most clinical form in children are meningitis and meningoencephalitis In this case report we described an eight year old girl with neurobrucellosis primarily displaying neuropsychiatric symptoms In endemic areas neurobrucellosis should be suspected in the evaluation of patients with unexplained neurologic symptoms Turk Arch Ped 2009; 44: 103 5 Key words: Brucellosis childhood neurobrucellosis neuropsychiatric manifestation
