Orofarenkste ve Blom-Singer ses protezi üzerinde üreyen Candida kolonizasyonu arasındaki ilişki ve bunun protez ömrü üzerine etkisi

Am a ç : Orofarenksteki Candida kolonizasyonunun , Blom-Singer ses protezi üzerindeki Candida kolonizas- yonuna ve protezin ömrü üzerine etkisi arafltırıldı. Hastalar ve Yöntemler: Total larenjektomi sonrası dü- flük basınçlı Blom-Singer ses protezi takılan, ameliyat sonrası dönemde radyoterapi görmüfl ve nistatin kul- lanmayan, ses protezinin bozulması nedeniyle değiflti- rilmesi için baflvuran 26 erkek olgu çalıflmaya alındı. Olguların orofarenksinden ve çıkarı- lan ses protezlerinin özofajeal ucundan alınan mantar kültürlerinde Candida kolonizasyonu arafltırıldı. Bulgular : Yirmi olgunun %77 protez, 14 olgunun %54 orofarenks kültüründe Candida kolonizasyonu görüldü. Orofarenksinde Candida olan tüm olgularda, protez üzerinde de Candida kolonizasyonu belirlendi. Ortalama pro- tez ömrü orofarenks kültüründe Candida üreyen olgular- da 5.7 ay dağılım 4-9 ay , üremeyenlerde ise 6.2 ay dağılım 3-10 ay bulundu. İki grup arasında istatistiksel olarak anlamlı farklılık bulunmadı p>0.05 . Sonuç: Orofarenkste saptanan Candida kolonizasyonunun protez ömrü üzerine etkisi olmadığı sonucuna varıldı

The relationship between the presence of Candida colonization in the oropharynx and on the Blom-Singer voice prosthesis and its effect on the length of prosthesis use

Objectives: We investigated the effect of oropharyn- geal Candida colonization on the contamination of the Blom-Singer voice prosthesis and its lifetime.Patients and Methods: The study included 26 male laryn- gectomized patients age range 45 to 76 years in whom the Blom-Singer low-pressure prosthetic valve inserted required removal due to improper functioning. AH patients had post- operative radiotherapy. Patients who had nystatin therapy were excluded. At the time of removal, cultures were obtained from the esophageal end of the prosthesis and from the oropharynx to be examined with regard to Candida grovvth.Results: Candida colonization was recovered from the cultures obtained on 20 prostheses 77% and from 14 oropharyngeal specimens 54% . The presence of oropha- ryngeal colonization was alvvays associated with that on prostheses. The average lengths of prosthesis use were 5.7 months range 4 to 9 months and 6.2 months range 3 to 10 months in patients with and vvithout positive oropha­ ryngeal Candida colonization, respectively p>0.05 .Conclusion: The results suggest that the presence of oropharyngeal Candida colonization has no effect on the prosthesis lifetime.
