Kronik farenjitli hastalarda psikiyatrik hastalık profili

Amaç: Kronik farenjit tanısı ile takip edilen hastalarda psikopatolojik yapı araştırıldı. Hastalar ve Yöntemler: En az altı aydır kronik farenjit semptomları olan, kulak burun boğaz muayenesinde ve incelemelerinde akut enfeksiyon tablosuolmayan 27 hasta çalışmaya alındı. Kontrolgrubunu kronik farenjit semptom ve bulguları olmayan ve daha önce hiç psikiyatrik tanı almamış 27 kişi 23 kadın, 4 erkek; ort. yaş 41.8±11.9 oluşturdu.Tüm hastalara ve kontrol grubuna psikiyatri hekimitarafından DSM-IV tanı ölçütlerine göre ruhsal muayene ve yapısal klinik görüşme Structural ClinicalInterview-SCID uygulandı.Bulgular: Tüm hastalarda eksen I’e göre psikiyatrikbozukluk saptandı. En sık saptanan patolojiler, sekizhastada %29.6 somatizasyon bozukluğu, sekiz hastada distimik bozukluktu. Kontrol grubunda da beş kişiye %15.6 psikiyatrik tanı kondu. Kronik farenjitlihastalarda psikiyatrik patoloji 6.4 kat fazla bulundu p=0.00, güven aralığı 2.86-14.31 . Hasta ve kontrolgrupları arasında eğitim düzeyi ve meslek açısındananlamlı fark yoktu sırasıyla, p=0.263 ve p=0.362 .Kronik farenjiti etkileyen alkol, sigara kullanımı, KBBmuayenesinde eşlik eden patolojiler, genel hastalıklarve ameliyat öyküsü gibi risk faktörleri açısından ikigrup arasında anlamlı fark saptanmadı. Sonuç: Kronik farenjit tanısıyla kulak burun boğazhekimlerine defalarca başvuran ve yakınmaları geçmeyen hastaların, psikiyatri hekimleriyle birlikte değerlendirilmesi ve tedavi edilmesini öneriyoruz

Psychiatric disorder profile in patients with chronic pharyngitis

Objectives: We investigated the psychopathological condition of patients with chronic pharyngitis.Patients and Methods: The study included 27 patients 23 females, 4 males; mean age 41.7±10.4 years; range 21 to 63 years who had persistent symp- toms of chronic pharyngitis for more than six months, without any evidence for an acute infection on ENT examination and routine tests. Twenty-seven healthy subjects 23 females, 4 males; mean age 41,8±11.9 years without any symptoms of chronic pharyngitis and a previous diagnosis of a psychiatric disorder formed the control group. AH the patients and Controls were assessed by a psychiatrist with the use of the Structural Clinical lnterviewfor DSM-IV Disorders SCID .Results: AH the patients were diagnosed to have Axis I psychopathology. The most frequent pathologies were somatisation disorder n=8,29.6% and dysthymic dis­ order n=8 . Five individuals 15.6% in the control group received psychiatric diagnoses. Psychiatric dis­ orders were 6.4 times more frequent in the patient group p=0.00, Cl=2.86-14.31 . No significant differ- ences were observed between the patient and control groups with respect to educational and occupational levels p=0.263 and p=0.362 , respectively .Comparisons with respect to risk factors associated with chronic pharyngitis showed no significant differ- ences between the two groups.Conclusion: We suggest that patients frequently presenting to ENT departments with repeated symp­ toms of chronic pharyngitis should also receive psy­ chiatric assessment and support.
