Olgu sunumu: Memenin paget hastalığı

Paget hastalığı (PH) tüm meme kanserlerinin yüzde 3ten azını oluşturan ve sıklıkla meme dokusunda altta yatan bir in situ veya invaziv karsinomla birliktelik gösteren meme başı-areola bölgesinin nadir bir malinitesidir. Klinik olarak PH meme başı-areola bölgesinde malin ülserleşme veya kabuklanmanın yanı sıra, sürekli ağrı veya kaşıntının eşlik ettiği kronik egzamalı değişiklikler ile kendini göstermektedir. Memede kitle veya mammografide anormal bulgular eşlik edebilir. Hastalığın erken dönem bulguları benin cilt lezyonlarına benzediği ve nadir görülen bir meme kanseri türü olduğu için tanı kolaylıkla atlanabilir. Tanının erken konulması hastaya total mastektomi yerine meme koruyucu cerrahi ile daha uzun süre kaliteli yaşam şansı sağlayacaktır. Bu olgu sunumunda, birinci basamak hekimlerinin konuyla ilgili farkındalığını artırmak amacıyla, ilk belirtilerden 2 yıl sonra tanı konan, sol meme Paget hastalığı ve intraduktal karsinomu olan 55 yaşındaki bir kadın hasta sunulmuştur.

Case report: Mammarian paget's diseases

Paget disease (PD) is a rare malignancy of the nipple-areola complex, comprising less than 3 percent of all breast cancers and often associated with an underlying in situ or invasive carcinoma in the breast parenchyma. Clinically, PD is manifested by chronic eczematous changes with malignant crusting or ulceration of the nipple-areola complex and persistent soreness or itching. There may be an associated breast mass or mammographic abnormality. As it is a rare condition and presenting symptoms can be mixed with benign skin lesions, it can easily be misdiagnosed. If it is diagnosed early, breast-conserving surgery can be performed instead of total mastectomy and this will increase the patients quality of life. In this case report, a 55-year- old woman with left breast Paget's disease and intraductal carcinoma is presented in order to raise the awareness of primary care physicians about Pagets diasease, which was diagnosed after 2 years after the initial symptom.
