Hacettepe Üniversitesi Tıp Fakültesi dönem dört öğrencilerinin uyku bozukluklarının ve uykululuk hallerinin Epworth uykululuk ölçeği ile değerlendirilmesi

Amaç: Hacettepe Üniversitesi Tıp Fakültesi (HÜTF) dönem dört öğrencilerinin bazı sosyodemografik özelliklerinin, gündüz uykululuk durumu sıklığının, uyku bozukluğuna yol açan kimi özelliklerin saptanması amaçlanmıştır. Gereç ve Yöntem: Araştırma evrenini dönem dört öğrencisi 374 kişi oluşturmuştur. Araştırmaya katılım yüzde 82.1 oranında gerçekleşmiştir. Bu çalışma kesitsel tipte bir epidemiyolojik araştırmadır. Genel uykululuk düzeyini değerlendirmede kullanılan Epworth Uykululuk Ölçeği (EUÖ) puanı araştırmanın bağımlı değişkenini; öğrencilerin sosyodemografik özellikleri, yattıkları ortamın fiziksel koşulları ve alışkanlıkları ise bağımsız değişkenlerini oluşturmuştur. Çalışma 5-6 Nisan 2OO5'te yapılmıştır. Bulgular: HÜTF dönem dört öğrencilerinin yaş ortalaması 21.9 olarak bulunmuştur (s.s.=0.9, altüst sınır=20-25, ortanca=22). Katılanların yüzde 53.1'i erkek, yüzde 46.9'u kadındır. Katılımcılardan yüzde 22.8'i EUÖ'de 11 ve üstü puan almıştır. EUÖ puanı ile; "15 gündür çökkün, ilgisiz, isteksiz hissetme", "son 15 gündür endişeli, bunaltılı ve gergin hissetme", "gece uyudukları odada yalnız kalamama", "gündüz uyuma" ve "uyuma ile ilgili sorunu olduğunu düşünme" arasında istatistiksel olarak anlamlı bir ilişki bulunmuştur (p'ler sırası ile; 0.016, 0.044, 0.014, 0.001, 0.000). Ortalama gece uyku süresi 7.2 saattir (s.s=1.2, alt-üst sınır=3-10, ortanca=7). Yüzde 54.4'ünün uyku ile ilgili sorunları olduğunu düşündükleri, yüzde 81.8'inin tıp fakültesi öğrencisi olmalarının uyku düzenlerini olumsuz etkilediğini düşündükleri belirlenmiştir. Tartışma: Öğrencilerin beşte birinden fazlasında EUÖ'ye göre gündüz uykululuk riski saptanmıştır. Bu konuda gerekli düzenlemelerin yapılması, öğrencilere ait barınma yerlerinde uyudukları odalarda yalnız kalmalarının sağlanması, tıp öğrencilerine bireysel rehberlik ve psikolojik danışmanlık hizmetlerinin etkin bir biçimde sağlanması yararlı olacaktır.

The evaluation of sleep disorders and sleepiness state with Epworth sleepiness scale of phase four students at Hacettepe University Faculty of Medicine

Objective: To determine some of the sociodemographic features, the frequency of daytime sleepiness state and some factors leading to sleep disorders among the phase four students of Hacettepe University Faculty. Material and Methods: The universe of the study was composed of 374 phase four students. The rate of participation in the study was 82.1%.The study was a cross-sectional epidemiological study. Epworth Sleepiness Scale (ESS) score was the dependent variable and sociodemographic features and habits of the students and the physical conditions of the place where they sleep made up the independent variables. The study was conducted on April 5-6, 2005. Results: The average age of the students was 21.9 (S.S=0.9, Min-Max=20-25, Median=22). Of the participants, 53.1% were male and 46.9% were female. Of the students, 22.8% got a score of 11 or more from ESS. There were statistically significant relationships between "ESS score" and "feeling depressed, indifferent, reluctant for the last 15 days", "feeling anxiety, suffocated, tense for the last 15 days", "not able to sleep alone in the bedroom", "daytime sleeps"and "presence of a sleep problem according to himself/herself" (p values were; 0.016, 0.044, 0.014, 0.001, 0.000, respectively). The average overnight sleeping time was 7.2 hours (S.D. =1.2, Min-Max=3-10, Median=7). Of the participants, 54.4% thought that they had sleeping problems and 81.8% thought that being a medical student affected their sleep negatively. Discussion: In more than one fifth of the students there was day-time sleepiness according to ESS. There should be some arrangements to overcome this issue. The students should sleep alone (stay in single room) in their accomodation. They need effective counselling services personally and psychologically.
