Endodontik tedavili molar dişlerin, endokron ile protetik rehabilitasyonu: Olgu serisi

Endodontik tedavi sonrası kron harabiyetinin arttığı molar dişlerin protetik rehabilitasyonunda, direkt ve indirekt restorasyonlar dahil olmak üzere farklı konvansiyonel tedavi yöntemleri uygulanabilir. Bilgisayar destekli tasarım/bilgisayar destekli üretim (CAD/CAM) ve adeziv teknolojideki gelişmeler ile birlikte endokron restorasyonlar konvansiyonel tedavilere alternatif olarak gösterilmiştir. Endokronlar, kor yapısı ve kron restorasyonunu bünyesinde birleştiren, çoğunlukla pulpa odasından, gerektiğinde de kök kanallarından tutuculuk sağlayarak (makroretansiyon), adezivlerle yeterli stabiliteyi elde eden (mikroretansiyon) monoblok restorasyonlardır. Bu olgu sunumunda kanal tedavili dişlerin endokron restorasyonları ile rehabilitasyonu dört farklı vakada ele alınmıştır. Vakalardan üçü, CAD/CAM sistemi ile lityum disilikat seramik bloklar kullanılarak üretilen endokron restorasyonlarını; bir vaka ise laboratuvarda indirekt kompozit rezin kullanılarak üretilen molar endokron restorasyonunu açıklamaktadır. Bu olgu sunumunun amacı; madde kaybı fazla olan kanal tedavili dişlerdeki sağlıklı diş dokusunun ve kanal dolgusunun korunmasını sağlayan endokron restorasyonların aşamalarının anlatılması ve klinik takip sonuçlarının aktarılmasıdır.

Prosthodontic rehabilitation of endodontically treated molar teeth with endocrown: The case series

After endodontic treatment, different conventional treatment methods can be applied in prosthetic rehabilitation of molar teeth, including direct and indirect restorations. Thanks to the help of computer aided design / computer aided manufacturing (CAD / CAM) and developments in adhesive technology, endocrown restorations have shown an alternative to conventional systems. Endocrowns are monobloc restorations that combine the restoration of the core structure and crown restoration, often by providing stabilization from the pulp chamber and from the root canals (macroretansion) to obtain adequate stability with adhesives (microretansion) when needed. In this case report, endocrown restoration and rehabilitation of endodontically treated teeth were discussed in four different cases. Three of the cases describe the molar endocrown restorations produced using lithium disilicate ceramic with CAD/CAM, and one case describes the molar endocrown restoration produced using indirect composite resin in the laboratory. The purpose of this case report is; to explain the stages of endocrown restorations which provide protection of healthy tooth tissue and canal filling in the endodontically treated teeth, and to report the clinical follow-up results.
