Mekonyum aspirasyon sendromu

SüleymanDemirel Üniversitesi TIP FAKÜLTESİ DERGİSİ: 2006 Mart; 13(1) Mekonyum aspirasyon sendromu Hasan Çetin, Hicran Altın Özet Mekonyum aspirasyon sendromu (MAS), zamanında doğan yenidoğan bebekler için önemli mortalite ve morbidite sebebidir. Mekonyum aspirasyon sendromu, havayollarının tam tıkanması ile atelektazi, parsiyel tıkanması ile havalanma artışı ve ödem, kimyasal pnömoni ve sürfaktan disfonksiyonu ile karekterizedir. Persistan pulmoner hipertansiyon mekonyum aspirasyon sendromunun önemli bir komplikasyonudur. Hemodinamik bozukluklar ve fetal hipoksi, intrauterin mekonyum pasajına yol açarak, aspirasyonun doğumdan önce ya da doğum sırasında meydana gelmesine neden olur. Ağız içinde yeşil sarı renkli sekresyon, takipne, interkostal retraksiyon, siyanoz, akciğerde aşırı hava tutulumu, akciğerde raller MAS.ın klinik semptom ve bulgulardır. MAS.ın tedavisi temel olarak hava yollarının mekonyumdan temizlenmesi, sürfaktan uygulanması ve destek tedavisine dayanmaktadır. Anahtar kelımeler: Mekonyum aspirasyon sendromu, yenidoğan, tedavi Abstract Meconium aspiration syndrome Meconium aspiration syndrome (MAS) is an important cause of morbidity and mortality in full term neonates. Meconium aspiration syndrome is characterized by the atelectasis due to the complete airway obstruction, emphysema and air leak syndrome resulted from the partial obstruction of airway, chemical pneumonitis, and surfactant dysfunction. Persistent pulmonary hypertension is major complication of meconium aspiration syndrome. Haemodynamic disturbances and fetal hypoxia may lead to intrauterine meconium emission that can be aspirated by the fetus in antenatal period. Clinical signs and symptoms of MAS include yellow green secretions from the mouth, tachypnea, intercostal retraction, cyanosis, overinflated chest due to air trapping, rales in the lung. Treatment of MAS should be based on clearing the meconium from the airway, surfactant supplementation and supportive care. Key words: Meconium aspiration syndrome, newborn, treatment.
